Welcome<br />
Welcome participants and introduce your guest speaker.<br />
Session Overview<br />
Say: “At the end of this session, you will be able <strong>to</strong>:<br />
List the ABCs of diabetes control (A1C, blood pressure, and cholesterol)<br />
Discuss why controlling the ABCs is important<br />
List steps <strong>to</strong> achieving healthier ABC levels<br />
NOTE: If you are conducting Session 6 in a single 90-minute meeting, skip ahead NOW <strong>to</strong> Part 2 Discussion<br />
Points List page when fnal formatting is completed). If the session is being held in three 60-minute meetings,<br />
proceed through this lesson plan as written.<br />
Review of the Previous Session<br />
Say: “In the last session, we learned a lot of facts about diabetes. Let’s see how everyone did with their<br />
pledges. Before we get started with <strong>to</strong>day’s session, let’s see how everyone did last week.”<br />
Check-In: Weekly Pledge and GAME PLAN Food and Activity<br />
Tracker; Barrier Busters<br />
Facilitate a discussion about how group members did with keeping their pledges and<br />
using their trackers. Also ask about the barriers they have faced since the last session. Ask for<br />
suggestions from group members on how <strong>to</strong> “bust” these barriers.<br />
After the discussion, say: “Let’s fll out our Individual Progress Charts. This is the time <strong>to</strong> award<br />
stickers for achieving goals, making progress, and needing a helping hand.<br />
Give each participant a new Weekly Pledge form and a new GAME PLAN Food and Activity<br />
Tracker.<br />
Part 2 Discussion Points<br />
Say: “If you have diabetes, take control of your health by knowing your ABCs. What do we mean by<br />
knowing your ABCs? We mean knowing your A1C level, your blood pressure level, and your<br />
cholesterol count. Maintaining normal levels of your A1C, blood pressure, and cholesterol is essential<br />
for remaining healthy if you have diabetes. If you don’t yourself have diabetes but know someone who<br />
does, share with them what we’ll be learning <strong>to</strong>day.”<br />
