V. SAFETY AND REGULATORY ISSUES<br />

The problem with consuming large doses of pure MCTs or their emulsions is the<br />

tendency to form ketone bodies (i.e., to induce metabolic acidosis). This outcome<br />

can be circumvented by using SLs or their emulsions. SL is safe <strong>and</strong> well tolerated<br />

in the body. Physiological <strong>and</strong> biochemical data suggest that SL emulsions, Intralipid<br />

20%, <strong>and</strong> fat emulsion 73403 (Kabi Pharmacia AB, Stockholm, Sweden), when fed<br />

to postoperative patients, were rapidly cleared <strong>and</strong> metabolized (130). The safety of<br />

Benefat was assessed, <strong>and</strong> no significant clinical effects were reported in subjects<br />

consuming up to 30 g/day (131). Other studies also indicate that SLs are safe (132).<br />
