Résumés :<br />
Nous allons nous trouver face à un bébé ou un très jeune enfant présentant une atteinte organique<br />
diagnostiquée, empêchant fortement toute stimulation visuelle. La première démarche<br />
de l’ophtalmologiste après le diagnostic précoce, est de mettre en place d’emblée un protocole<br />
de traitement chirurgical afin de préserver au maximum le capital visuel du bébé.<br />
L’accompagnement orthoptique va donc se situer tant dans l’aspect post-opératoire que dans<br />
l’accompagnement du développement visuel de l’enfant, à tous les niveaux d’interaction<br />
entre la vision et les autres sens. Il s’agira d’un accompagnement spécifique des parents dans<br />
la relation avec leur enfant malvoyant. L’orthoptiste se trouve face à une rééducation et une<br />
réadaptation précoce d’un bébé, puis d’un enfant malvoyant en constante construction et par<br />
conséquent avec un système visuel qui reste encore malléable et évolutif.<br />
We are going to present you a baby or a very young child, having a diagnosed organically pathology<br />
obstructing strongly the visual stimulation. Next to his premature diagnosis an important step for the<br />
ophthalmologist, is first to prescribe a operation treatment in order to preserve as much as possible the<br />
baby’s visual capital. The orthoptic’s role is important as well in the post-operative aspect as in the<br />
visual development of the child, at all interaction levels between the sight and the other sense. The<br />
orthoptist will specifically help the parents in their relation with their visual impaired child. The<br />
orthoptist has to face a premature re education and re adaptation of a baby and then of a visual impaired<br />
child in a growing construction and therefore with a still malleable and increasing visual system.<br />

Cette conférence va rappeler que nous parlons d’un bébé, en pleine construction, pour<br />
lequel la vision est le principal accès au monde. Cette vision prévaut à toute autre<br />
information sur le plan neurophysiologique et dans la construction de l’inter-modalité<br />
(Y. Hatwell). Que va-t-il se passer pour un enfant atteint très précocement puisque<br />
porteur d’une cataracte congénitale bilatérale ?<br />
En quoi la chirurgie qui est très précoce, et l’accompagnement orthoptique qui se situera<br />
d’emblée en post-opératoire, vont être nécessaires au développement de cet enfant ?<br />
Quel vont être les modalités de la rééducation orthoptique ? Où se situe l’équipement<br />
optique dans le suivi et la rééducation orthoptique de l’enfant (travail de partenariat) ?<br />
Quelles observations pouvons-nous faire dans l’évolution globale de ces enfants à court<br />
et moyen terme ?<br />
Le bébé porteur d’une cataracte congénitale bilatérale se trouve dans la situation d’une<br />
forte perturbation de l’imagerie rétinienne, bilatérale. Cela aboutit à une amblyopie<br />
bilatérale à laquelle peuvent s’ajouter des perturbations motrices telles que le strabisme<br />
