(Refer to Attachment Seven.) For each reference, the offeror must provide the following<br />

information:<br />

• Contact Information. The offeror must provide a client contact name, title, phone number,<br />

email address, company name, and mailing address. The offeror also must include the same<br />

information for an alternate client contact, in case the State cannot reach the primary contact.<br />

Failure to provide this information or providing information that is inaccurate or out of date<br />

may result in the State not including the reference in the evaluation process or rejecting the<br />

offeror’s Proposal. The contact information given must be for a person within the client’s<br />

organization and not a co-worker or a contact within the offeror’s organization, subsidiaries,<br />

partnerships, etc.<br />

• Work Name. The offeror must provide the name or title for the work, such as a project name,<br />

from which it obtained the mandatory experience.<br />

• Dates of Experience. The offeror must complete this area with a beginning month and year<br />

and an ending month and year to show the length of time the offeror performed the work, not<br />

just the length of time the offeror was engaged by the reference.<br />

• Description of the Related Service Provided. The State will not assume that, since the<br />

experience requirement is provided at the top of the page, all descriptions on that page relate<br />

to that requirement. The offeror must reiterate the experience being described, including the<br />

capacity in which the work was performed and the role of the offeror on the Work. It is the<br />

offeror’s responsibility to customize the description to clearly substantiate the qualification.<br />

• Description of how the related service shows the offeror’s experience, capability, and<br />

capacity to develop the Deliverables and do the Work.<br />

The offeror must list each work experience separately and completely every time it is referenced,<br />

regardless of whether it is on the same or different pages of the form.<br />

Required Experience and Qualifications. The offeror must complete this section to<br />

demonstrate that it meets the requirements for experience. For each reference, the offeror must<br />

provide the information in the same manner as described under Mandatory Experience and<br />

Qualifications above.<br />

THE OFFEROR MAY NOT USE THE EXPERIENCE OR QUALIFICATIONS OF A SUBCONTRACTOR<br />

TO MEET ANY OF THE ABOVE MANDATORY QUALIFICATIONS OR EXPERIENCE. THESE MUST<br />

BE FULFILLED EXCLUSIVELY THROUGH THE QUALIFICATIONS AND EXPERIENCE OF THE<br />

OFFEROR. If the offeror seeks to meet any of the other qualifications and experience through a<br />

subcontractor, the offeror must identify the subcontractor by name in the appropriate part of the<br />

Offeror Profile Summary Form, in Attachment Seven to this <strong>RFP</strong>, for each reference.<br />

Personnel Profile Summary Forms. Each Proposal must include a profile for each key member of the<br />

proposed work team. This <strong>RFP</strong> includes Personnel Profile Summary Forms as Attachment Eight, and the<br />

offeror must use these forms and fill them out completely for each reference.<br />

The Personnel Profile Summary Forms contained in this <strong>RFP</strong> have been customized for the<br />

applicable candidate requirements. Each page of the forms may contain minor variations. If an<br />

offeror elects to duplicate the forms electronically, the offeror must carefully review each form to<br />

ensure that it has been copied accurately. Failure to duplicate the forms exactly may lead to the<br />

rejection of the offeror’s Proposal.<br />

The offeror must propose a Work team that collectively meets all the requirements in this <strong>RFP</strong>, as<br />

demonstrated through the Personnel Profile Summary Forms. Additionally, each team member may<br />

have requirements listed in this <strong>RFP</strong> that the team member must individually meet. The offeror must<br />

name all candidates proposed, and each must meet the technical experience for the candidate's position.<br />

The State will not consider a candidate’s overlapping months of experience toward meeting the<br />

experience requirements in this <strong>RFP</strong>. Therefore, for each requirement for a key position, the Personnel<br />
