OBF Forms Translation Questionnaire (TQ) Use the TQ form to identify which and how CICs, SACs, and NXXs will be routed by the trunks being ordered. You should submit the TQ form when local, Feature Group B (FGB), or Feature Group D (FGD) translations/routing is required for new, change, disconnect or activation activities. You will need multiple ASRs if the TQ is associated with two or more requests. The TQ will be associated with one of the ASRs as an additional form and carries the purchase order number (PON) of the associated ASRs. The ASRs that do not have the TQ attached should have the PON number of the master ASR to complete the association of all involved ASRs. Transport Formerly called Special Access, the Transport form provides all the information for ordering services between an access customer terminal location (ACTL) and an end user location such as special access service, switched access facility, or unbundled transport. The form is divided into two sections: • Circuit Detail section provides entries for specifying ordering options, transmission levels, general exchange tariff options, and for registration requirements. • Location section provides fields for describing the termination of the special access service at a customer location, switched access facility, or transport at a customer location. This section includes fields that may be needed for gaining access for installation purposes. The steps for completing a Transport request are: 1. Complete the ASR form. 2. Complete the Transport form. 3. Complete the Circuit ID information. A-12 4.8.0 User Guide April 2009
OBF Forms WATS This form is used by the customer to order WATS access line. The WATS Access Line (WAL) form must always be associated with an ASR Form, which contains administrative and bill detail necessary for the provisioning of this request. The field entries contained within the WAL form are provided by the customer. All information required for ordering a WATS Access Line is provided for in the various fields contained within the WAL Form. The Circuit Detail Section provides entries for the specification of ordering options, transmission levels, hunting requirements, General Exchange Tariff options and for registration requirements. The steps for completing a WATS form are: 1. Complete the ASR form. 2. Complete the WAL form. 3. Complete the MSL form 4. If needed, complete ACI form 5. If needed, complete Multi-EC form Trunking The Trunking form provides information about ordering options, transmission levels, special routing, and service class routing, as well as information about secondary locations and serving areas for tandem, end offices, or signalling points. Trunking is a subsection of the TQ form. It is required for all trunking/routing/ translation requests and is used in addition to the Administrative and Common sections for FGD and local service only.
