- Case management<br />
- Health care, mental health care, or psychiatric care<br />
- Transportation<br />
- Employment assistance<br />
- Education<br />
- Vocational opportunities<br />
- Life skills training<br />
- Counseling, including housing counseling<br />
- Substance abuse treatment<br />
- Parenting skills<br />
- Budgeting<br />
- <strong>Housing</strong> information referrals<br />
- If services are provided, whether each tenant will have an Individual Service Plan (ISP) that<br />
details the specific services to be provided, their frequency and duration.<br />
- Target Population. In the event the number of PBV applications exceeds the number of<br />
available vouchers, projects will be prioritized as follows:<br />
- Existing housing projects will be favored over new construction or rehabilitation projects.<br />
- Project provides comprehensive supportive services.<br />
- Project serves homeless families with children (2+ bedroom units).<br />
- Project serves homeless persons with disabilities.<br />
- Project serves homeless elderly persons.<br />
- Project serves homeless individuals.<br />
- Project serves non-homeless persons with disabilities.<br />
- Project serves non-homeless elderly persons.<br />
Applications need a minimum score of 75 points (out of 100) to receive an allocation of Projectbased<br />
Vouchers.<br />
4. <strong>Housing</strong> <strong>Commission</strong> Owned Units and Units Owned by the <strong>Housing</strong><br />
<strong>Commission</strong>’s Limited Liability Companies<br />
Project-based units in SDHC owned properties shall adhere to the same policies and procedures<br />
described in this <strong>Chapter</strong> with the exception of those referenced in this section.<br />
Project Selection<br />
Project-Based assistance for SDHC-owned properties will not be competitively bid, nor will there be<br />
an approval process through HUD. Rather, SDHC can make Project-Based funding available in SDHC<br />
owned properties in response to a written request. A separate request is required per property and<br />
must include the following: name and address of the property, total number of units, requested<br />
number of Project-Based vouchers, number of vouchers requested per unit size, proposed rent per<br />
