<strong>Chapter</strong> 14 – Self-Sufficiency Action Plan<br />

SDHC policies and procedures regarding accommodations to a person with a disability as contained<br />

in the Section 8 Administrative Plan, described in <strong>Chapter</strong> 1 Section F, shall be the adopted policy<br />

and procedures for the FSS Program.<br />

Additionally, SDHC policies and procedures pertaining to verification of reasonable accommodation<br />

to accommodate a person with a disability, as contained in the Section 8 Administrative Plan<br />

<strong>Chapter</strong> 3 Section N, shall be the adopted policy and procedures for the FSS Program.<br />

5. Denial of Participation<br />

The SDHC may deny participation to families who wish to join the program. Participation will be<br />

denied to families who:<br />

- Owe money to SDHC or any other housing authority;<br />

- Have support service needs which present an undue financial or administrative burden to the<br />

Agency;<br />

- Act in ways which are contrary to the intent of the program (e.g., repeatedly fail to attend<br />

intake appointments; fail to attend mandatory activities; fail to create, update or implement a<br />

Career Plan; communicate a lack of intent to engage in activities related to successful<br />

completion of the Contract; perform inappropriate actions or communications toward FSS<br />

participants, staff or its partners);<br />

- Have successfully met the terms of a previous FSS Contract with the SDHC or another housing<br />

authority, and officially graduated from the program with or without an escrow account;<br />

- Have participated in the FSS Program previously, through the SDHC or another housing<br />

authority, but did not meet graduation requirements during their original contract;<br />

- Do not meet eligibility criteria as detailed in section B. Eligibility For Participation above, which<br />

states "Participation in the FSS Program is available on a voluntary basis.”<br />

D. SERVICE DELIVERY [24 CFR 984.303(a), 984.303(b)]<br />

Once enrolling and after attending a mandatory orientation session, the individual is invited to schedule<br />

an intake appointment with an FSS Service Coordinator.<br />

1. Contract of Participation<br />

During the intake appointment, a Contract is signed by the designated participant. The Contract<br />

describes the rights and responsibilities of the participant, SDHC, and its partners. The initial<br />

Contract term is five (5) years. The contract may be extended, in writing, and at the family’s request,<br />

for a maximum of two additional years for good cause. Approval of such request is at the discretion<br />

of a Program Supervisor. Extensions will only be granted in rare circumstances, beyond the control<br />

of the family, and which prevent completion of the Career Plan.<br />

The FSS Program start date will be the first day of the month following the date in which the<br />

Contract was signed by the participant and the FSS Service Coordinator. In compliance with HUD<br />

regulations, a contract cannot be executed if the time between a family’s most recent HUD 50058<br />

recertification and the FSS Contract start date exceeds 120 days. When a HUD 50058 recertification<br />

exceeds the 120 day time period, the family will have to request an interim 50058 recertification<br />

with the appropriate SDHC housing assistance department.<br />
