S2Comment: Dermatomes are regions of the body to which specificnerve roots predominantly distribute their primary afferent fibers;they are based on the corresponding spinal cord segments thatreceive the primary sensory information through the dorsal roots. Inthe spinal cord, sensory fibers from a specific dermatomal regionactually distribute to at least three segments of the spinal cord,including the predominant segment, and a segment above andbelow. Thus, a destructive lesion of one nerve root does not leavethe dermatome totally devoid of sensation; that phenomenon wouldrequire destruction of the specific associated nerve root, as wellas the nerve roots above and below that dermatome. However,an irritative lesion, such as the impingement of a herniated disc(nucleus pulposus) on a dorsal root, will produce sharp, radiatingpain in the distribution of the principal dermatome. The nonuniformdistribution of dermatomes across the body reflects the rotation ofthe limb buds during development. Knowledge of the dermatomes isimportant for distinguishing between nerve root lesions (which mayindividually produce diminished sensation in a region of the body)and a peripheral nerve lesion (which may produce total anesthesia inthe distribution of that nerve).Regional Neuroscience See book 9.15
