Inferior olivary nucleusComment: At this level, the primary sensory somatosensory nuclei,gracilis and cuneatus, are giving rise to the internal arcuate fibers,which cross the midline to form the medial lemniscus, found justdorsal to the medullary pyramids. At this point, the secondary sensoryaxons from both the epicritic system (medial lemniscus [ML]) and fastprotopathic system (spinothalamic tract [STT]) are crossed, but areseparated physically throughout the medulla, and can be separatelyinvolved in pathology (such as a paramedian stroke affecting theML but not the STT). Damage results in contralateral loss of fine,discriminative touch; joint position sense; vibratory sensation (ML);or pain and temperature sensation (STT). Secondary sensory (andmany higher order) axons carrying sensation for excruciating paintravel in the spino-reticular system and are represented bilaterally andmore diffusely than the STT, accounting for the inability of isolatedtractotomies to alleviate intractable pain permanently.Regional Neuroscience See book 11.3
