RESUMEN<br />
La Insuficiencia <strong>Renal</strong> Crónica Terminal (IRCT) es un grave problema sanitario mundial cuya<br />
mortalidad es del 100% si no es tratada y su morbilidad coasociada es alta. Hay dos formas de tratarla<br />
llamadas terapias de sustitución renal (TSR): diálisis y trasplante renal. El autor se propone evaluar los<br />
costos asociados a estas dos intervenciones económicas para dilucidar cuál es la más costo-efectiva<br />
(ACE) en Venezuela al presente; posteriormente, vistos los cambios económicos recientes y<br />
congruente con la Teoría General de Sistemas y la Teoría de las Contingencias, establecerá el impacto<br />
socio-económico sobre el Gasto Público en Salud del país y creará propuestas al respecto. Para la<br />
muestra poblacional proporcional se tomaron en cuenta los costos directos e indirectos de dos<br />
modalidades de cada una: la hemodiálisis y peritoneodiálisis, para la diálisis, y para el trasplante, el<br />
de cadáver y de vivo, así como los años de vida ajustados por calidad (AVAC). Se revisó la bibliografía<br />
mundial relacionada, en especial de los países donde se hacen más trasplantes, así también la<br />
nacional, datos gubernamentales y de ONG’s. Observó como otros autores, que la intervención más<br />
costo-efectiva hoy día y con mayor énfasis en la calidad de vida es el trasplante renal. Se analizó la<br />
data donde destaca el importante impacto negativo al Gasto Público en Salud de la política actual<br />
aplicada a los TSR en Venezuela. Finalmente, se hacen conclusiones y recomendaciones específicas<br />
para el ámbito nacional venezolano, con el objetivo de maximizar resultados y adaptar el gasto<br />
público de salud a la realidad social y económica actual.<br />

ABSTRACT<br />
The end-stage renal disease (ESRD) is a major global health problem whose mortality is 100% if it is not<br />
treated and its related co-morbidities are high. There are two ways to deal with it so called renal<br />
replacement therapies (RRT): dialysis and kidney transplantation. The author intends to assess the costs<br />
associated with these two economic interventions to clarify what is the most cost-effective (ACE) in<br />
Venezuela currently; then seen recent economic changes and consistent with the general theory of<br />
systems and the theory of the contingencies, establish the socio-economic impact on the public<br />
expenditure on health in the country and create proposals in this regard. Proportional population sample<br />
took into account the direct and indirect costs of two modalities of each: hemodialysis and<br />
peritoneodiálisis, dialysis, and transplantation, cadaveric and live, as well as the quality-adjusted life years<br />
(QALYS). Revised World bibliography relating, in particular in countries where more transplants are made,<br />
and also the national, governmental and NGO data's. Noted as other authors, that the intervention most<br />
cost-effective today and with more emphasis on the quality of life is kidney transplantation. Analyzed data<br />
where highlights the significant negative impact to the public expenditure on health of current policy to<br />
the TSR in Venezuela. Finally, make findings and recommendations specific to Venezuelan national level<br />
aiming to maximize results and adapt health expenditure in the social and economic realities.<br />
