Documentation revealed that<strong>of</strong> the 40,058 NVDs that had been identified as issued to the ISF, more than 26,000lacked supporting transfer documentation detailing transfer date, serial number, <strong>and</strong>receiving Iraqi <strong>of</strong>ficial, among other information.The MNSTC-I internal review was continuing at the conclusion <strong>of</strong> our fieldwork.Office <strong>of</strong> Inspector General, Department <strong>of</strong> Defense, AssessmentD2008-D00SPO-0271.000On August 19, 2008, the Office <strong>of</strong> Inspector General announced an “Assessment <strong>of</strong> theAccountability <strong>of</strong> Night Vision Devices Provided to the <strong>Security</strong> Forces <strong>of</strong> Iraq.” Thiswork began immediately. Specifically, the DoD IG assessment was to determine whetherthe accountability, control, <strong>and</strong> physical security over the distribution <strong>of</strong> NVDs providedto the ISF was adequate. The assessment included the areas <strong>of</strong> contracting <strong>and</strong>procurement, transportation <strong>and</strong> storage, U.S. issuance procedures to the ISF, <strong>and</strong>oversight support subsequently provided by the ISF.Recommendations, Client Comments, <strong>and</strong> OurResponse4.a. We recommend that Comm<strong>and</strong>er, Multi-National <strong>Security</strong> Transition Comm<strong>and</strong>-Iraq reconcile night vision devices discrepancies at Taji National Depot betweeninventory data <strong>and</strong> stocks on-h<strong>and</strong> consistent with End-Use Monitoring requirements.Client CommentsMNSTC-I concurred. The Comm<strong>and</strong>er, MNSTC-I stated that action on thisrecommendation was complete.Our ResponseComm<strong>and</strong>er, MNSTC-I comments were partially responsive. While MNSTC-Iconcurred with the recommendation, its comments failed to describe actions taken toaccomplish the recommendations <strong>and</strong> show the completion dates <strong>of</strong> those actions. Werequest that Comm<strong>and</strong>er, MNSTC-I provide this information in reply to the final report.4.b. We recommend that Comm<strong>and</strong>er, Multi-National <strong>Security</strong> Transition Comm<strong>and</strong>-Iraq mentor Iraq <strong>Security</strong> Forces personnel to develop a suitable system foraccountability <strong>of</strong> night vision devices in accordance with signed agreements <strong>and</strong> theForeign <strong>Assistance</strong> Act section 505.55
