Stem Cell Reinfusion Over Two Consecutive Days,<br />

Plus Delayed G-CSF, Hastens Engraftment<br />

ABSTRACT<br />

Outpatient reinfusion of mobilized peripheral stem cells over 2 consecutive<br />

days can minimize the side effects associated with larger volumes, increased<br />

dimethylsulfoxide, <strong>and</strong> elevated numbers of mature cells in apheresis products.<br />

Engraftment in the first nine patients receiving split reinfusions was compared to<br />

20 previous single-reinfusion transplants. Both groups received the same number<br />

of CD34 +<br />

cells/kg, 4.5 ± 0.8 <strong>and</strong> 4.7 ± 2.7, <strong>and</strong> started granulocyte colonystimulating<br />

factor (G-CSF) 24 hours before reinfusion began. Results are expressed<br />

as the mean ± st<strong>and</strong>ard deviation. P values 500 13 + 2.6 11 ± 1.2 0.0168<br />

Fractionating stem cell reinfusion significantly reduced neutropenic days <strong>and</strong><br />

hastened myeloid engraftment. There was a trend toward faster platelet recovery.<br />

To further improve platelet recovery, G-CSF was delayed until 48 hours after<br />

completion of split reinfusion. CD34 +<br />
