The CD19-directed immunotoxin B43-PAP is a promising new agent against Blineage<br />

acute lymphoblastic leukemia (ALL). We have assessed the feasibility of<br />

using this agent in decreasing the pretransplant leukemic burden of pediatric<br />

patients undergoing allogeneic BMT. Seventeen high-risk remission ALL patients<br />

were treated with a 5-day B43-PAP pre-BMT window therapy (100 pg/kg/day X<br />

5 consecutive days), before initiation of the pretransplant conditioning. This<br />

treatment was generally well tolerated with no dose-limiting toxicity. The<br />

probability of remaining in remission at 3 years was 74 ± 11%. Eleven of the 17<br />

patients remain alive, free of leukemia at 1.8—4.5 years post-BMT, with a 3-year<br />

disease-free survival of 65 ± 12%.<br />
