AND PAIN INHIBITION IN OLDER ADULTS WITH AND<br />

WITHOUT KNEE OSTEOARTHRITIS<br />

Introduction: Quantitative sensory testing (QST) suggests that osteoarthritis<br />

patients demonstrate generalized hyperalgesia and impaired<br />

endogenous pain inhibitory capacity, but the correlates of these sensory<br />

alterations are poorly understood. The present investigation examines<br />

whether insomnia may be associated with hyperalgesia and reduced pain<br />

inhibition in osteoarthritis (OA).<br />

Methods: In this quasi-experimental study, we used polysomnography<br />

and knee radiography to carefully diagnose 4 groups of older adults according<br />

to research diagnostic criteria for sleep and knee osteoarthritis.<br />

We compared standardized indices of pressure pain threshold (PPT) and<br />

pain inhibition in 100 participants meeting criteria for: 1) Normal-Sleep/<br />

No-OA (n=22); 2) Normal-Sleep/+OA (n= 14); 3) Primary-Insomnia/<br />

No-OA (n=15) and 4) Insomnia/+OA (n=49). Baseline PPT was measured<br />

bilaterally via algometry at the trapezius. Pain inhibition was<br />

assessed via a diffuse noxious inhibitory control (DNIC) paradigm<br />

quantifying percent change in trapezius PPT from baseline relative to<br />

trapezius PPT assessed during contra lateral hand immersion in a 4°C,<br />

circulating ice water bath (DNIC index).<br />

Results: ANOVA s demonstrated a significant omnibus effect for group<br />

status on both PPT and DNIC (p
