Procedure<br />

The PHA's procedure for conducting annual recertifications will be to schedule the date and time<br />

of appointment and mail a notification to the family. The PHA may also elect to have the family<br />

complete a mail-in recertification.<br />

Completion of Annual Recertification<br />

The PHA will have all recertifications for families completed before the anniversary date. This<br />

includes notifying the family of any changes in rent at least 30 days before the scheduled date of<br />

the change in family rent.<br />

Persons with Disabilities<br />

Persons with disabilities who are unable to come to the PHA's office will be granted an<br />

accommodation by conducting the interview at the person's home or by mail, upon request by a<br />

family that the accommodation is needed. Verification may be required.<br />

Collection of Information<br />

The PHA has established appropriate recertification procedures necessary to ensure that the<br />

income data provided by families is complete and accurate.<br />

The PHA will require the family to complete an updated Participant Recertification Assessment<br />

Form prior to all recertification interviews.<br />

Requirements to Attend<br />

All adult household members will be required to attend the recertification interview. Minors<br />

who will turn 18 within four months after the appointment date are required to attend the<br />

recertification interview. All approved live-in aides must be present at the recertification<br />

interview, provide current identification, and provide proof of residency in the subsidized unit.<br />

Failure to Respond to Notification to Recertify<br />

The PHA must send written notification of the recertification appointment or mail-in due date. If<br />

the participant is unable to keep the first scheduled appointment or mail-in date a final<br />

appointment will be scheduled and written notification will be mailed. If the participant fails to<br />

comply with either the first appointment or final appointment date (mail in due date), the family<br />

will be proposed for termination and informed of their right to an informal hearing. Exceptions<br />

to these policies may be made by a Supervisor or Manager if the family is able to document an<br />

emergency situation that prevented them from completing the recertification or if requested as a<br />

reasonable accommodation for a person with a disability. Families will be notified in the letter<br />

that they will be permitted to reschedule for a documented emergency if they call before the<br />

missed appointment.<br />
