Written Third Party Verification (Tenant Provided Documentation) Level 4<br />

An original or authentic document generated by a third party source dated either within the 60-<br />

day period preceding the reexamination or PHA request date. Such documentation may be in the<br />

possession of the tenant (or applicant), and is commonly referred to as tenant-provided<br />

documents. The PHA may, at its discretion, reject any tenant-provided documents and follow up<br />

directly with the source to obtain necessary verification of information.<br />

The participant is required to supply any information requested by the PHA for use in a regularly<br />

scheduled annual or interim reexamination of family income and composition in accordance with<br />

HUD requirements. The PHA may determine that the tenant is not in compliance with program<br />

requirements and terminate tenancy or assistance, or both, if the tenant fails to provide the<br />

requested information within the time period requested by the PHA.<br />

Written Third-Party Verification (Traditional Third Party) Level 3<br />

Written third-party verification is used to verify information directly with the source. Third-party<br />

written verification forms will be sent and returned via first class mail, fax or email. The family<br />

will be required to sign an authorization for the information source to release the specified<br />

information.<br />

Verifications received electronically directly from the source are considered third party written<br />

verifications.<br />

Written third party verification returned more than 60 days from the processing date will be<br />

deemed late and will not be used in the verification process; such documents shall be forwarded<br />

to the client file. If written third-party verification is delayed or not possible, the PHA will<br />

attempt to obtain information through oral verification.<br />

Oral Third-Party Verification Level 2<br />

Oral third-party verification will be used when written third-party verification is not possible.<br />

When third-party oral verification is used, staff will be required to document with whom they<br />

spoke, the date of the conversation, and the facts provided. If provided by telephone, the PHA<br />

must originate the call.<br />

Tenant Declaration Level 1<br />

When verification cannot be made by third-party verification or review of documents, families<br />

will be required to submit a self-certification.<br />

Self-certification means a notarized statement or an affidavit under penalty of perjury.<br />

C. RELEASE OF INFORMATION [24 CFR 5.230]<br />

Adult family members, including spouse and/or co-head, regardless of age, will be required to<br />

sign the HUD 9886 Release of Information/Privacy Act form.<br />
