J. CERTIFICATION OF DOMESTIC VIOLENCE, DATING VIOLENCE OR<br />

STALKING (SECTION 606 AND 607 OF VAWA)<br />

The owner, manager, or PHA may request that an individual certify in writing via a HUD<br />

approved certification form that the individual is a victim of domestic violence, dating violence,<br />

or stalking. The certification form will be completed, signed and dated by the victim to certify<br />

he/she is the victim of domestic violence, dating violence, or stalking, and that the incident or<br />

incidences are bona fide incidences of such actual or threatened abuse. The victim shall include<br />

the name of the perpetrator. The individual shall provide such certification within 14 business<br />

days after the owner, manager, or public housing agency requests such certification.<br />

Other acceptable documentation in lieu of or in addition to the certification form:<br />

1) Federal, State, tribal, territorial, or local police or court record.<br />

2) Documentation signed and attested to by an employee, agent or volunteer of victim<br />

service providers, an attorney or a medical professional from whom the victim has sought<br />

assistance in addressing the domestic violence or abuse. The professional must attest<br />

under penalty of perjury (28U.S.C. 1746) that to their belief that the incident or incidents<br />

in question are bona fide incidents of abuse.<br />

K. CLAIMS OF IDENTITY THEFT:<br />

Families disputing information due to identity theft must provide documentation of their attempt<br />

to correct the misinformation. Documentation includes: Police report, FCC claim letter, Letter to<br />

erroneous reporting agency. Information must be provided within 30 days, of the re-examination<br />

or application, or the information being disputed may be used by the PHA.<br />
