Verification of Marital Status<br />

INSTRUCTION: This would be used to determine spouse for income and deduction and noncitizen<br />

purposes<br />

1) Verification of divorce status will be a certified copy of the divorce decree, signed by a<br />

Court Officer.<br />

2) Verification of a separation may be a copy of court-ordered maintenance or other records.<br />

3) Verification of marriage status is a marriage certificate.<br />

Familial Relationships<br />

The following verifications will always be required if applicable:<br />

Verification of relationship:<br />

1) Official identification showing names<br />

2) Birth Certificates<br />

3) Baptismal certificates<br />

Verification of guardianship is:<br />

1) Court-ordered assignment<br />

2) Verification from social services agency<br />

Verification of Permanent Absence of Family Member<br />

If an adult member who was formerly a member of the household is reported permanently absent<br />

by the family, the PHA will consider any of the following as verification:<br />

1) Husband or wife institutes divorce action.<br />

2) Husband or wife institutes legal separation.<br />

3) Order of protection/restraining order obtained by one family member against another.<br />

4) Proof of another home address, such as utility bills, canceled checks for rent, drivers<br />

license, or lease or rental agreement, if available.<br />

5) Statements from other agencies such as social services or a written statement from the<br />

landlord or manager that the adult family member is no longer living at that location.<br />

6) If the adult family member is incarcerated, a document from the Court or correctional<br />

facility should be obtained stating how long they will be incarcerated.<br />

7) If no other proof can be provided, the PHA will accept an affidavit from the head of<br />

household explaining the absence of a family member.<br />

Verification of Change in Family Composition<br />

The PHA may verify changes in family composition (either reported or unreported) through<br />

letters, telephone calls, utility records, inspections, landlords, neighbors, credit data, school or<br />

Bureau of Motor Vehicle (BMV) records, and other sources.<br />

Verification of Disability<br />

Verification of disability must be receipt of SSI or SSA disability payments under Section 223 of<br />

the Social Security Act or 102(7) of the Developmental Disabilities Assistance and Bill of Rights<br />

Act (42 U.S.C. 6001(7). In cases where a family member is temporarily disabled (i.e. workers<br />

compensation), verification from a physician will be accepted.<br />
