Cardiovascular<br />

C<br />

Specific to rural general practice<br />

<strong>The</strong> GP will demonstrate the ability to:<br />

• per<strong>for</strong>m advanced management of arrhythmias in<br />

a rural setting<br />

• diagnose, from history and ECG, with or without<br />

point-of-care blood testing, acute coronary syndrome<br />

and implement an appropriate course of action<br />

• care <strong>for</strong> a patient with a more severe exacerbation<br />

of congestive heart failure requiring admission to a<br />

rural hospital<br />

• manage cardiac arrest in children and adults,<br />

demonstrating familiarity with a defibrillator and<br />

appropriate airway management<br />

• make considered, rational decisions when discussing<br />

with the patient and their family/whānau whether<br />

to transfer to a base hospital <strong>for</strong> investigation and<br />

management, or to manage in the rural hospital<br />

• manage rural hospital in-patient care, rehabilitation,<br />

secondary prevention, discharge planning and<br />

appropriate follow-up.<br />

Professionalism<br />

<strong>The</strong> GP will demonstrate the ability to:<br />

• maintain accurate records, including in<strong>for</strong>mation<br />

relevant to cardiovascular risk assessment, results<br />

and medications<br />

• consider cardiovascular risk and disease in the context<br />

of the whole person, and advise the patient accordingly<br />

• ensure systems are in place to offer a cardiovascular<br />

screening service in the practice, and endeavour to<br />

meet targets set in the PHO Per<strong>for</strong>mance Programme<br />

• recognise serious cardiovascular symptoms and<br />

promptly initiate management and/or referral and ensure<br />

the patient is seen and referred within a safe timeframe<br />

• per<strong>for</strong>m prompt interpretation and action on test results<br />

• develop an understanding of the psychological and<br />

social impact of cardiovascular disease on patients<br />

and their families/whānau.<br />

Scholarship<br />

<strong>The</strong> GP will demonstrate the ability to:<br />

• undertake an audit of practice per<strong>for</strong>mance in<br />

detection and management of cardiovascular disease<br />

• teach members of the primary care team about<br />

cardiovascular disease, its significance and management<br />

• maintain up-to-date knowledge of management of<br />

acute and chronic cardiovascular conditions<br />

• undertake further study appropriate to their work in<br />

relation to CVD, such as long-term condition management.<br />
