C Cardiovascular<br />
Context of <strong>General</strong> <strong>Practice</strong><br />
<strong>The</strong> GP will demonstrate the ability to:<br />
• understand the demographics of their practice population<br />
to allow appropriate screening, organised or opportunistic,<br />
and awareness of prevalent conditions<br />
• manage the referral and admission pathways and<br />
protocols in their local area to ensure patients receive<br />
secondary care in an efficient and timely manner<br />
• consider what methods would help to reach their practice<br />
population <strong>for</strong> cardiovascular screening and improved risk<br />
factor modification and disease management<br />
• engage with other health care professionals, providing<br />
both support to reduce the risk of CVD and rehabilitation<br />
following a CV event.<br />
• consider the different members of the primary care<br />
team and how they can each participate fully in<br />
cardiovascular disease prevention, detection and<br />
management in their practice<br />
Management<br />
<strong>The</strong> GP will demonstrate the ability to:<br />
• set up systems to efficiently identify and contact patients,<br />
and deliver a cardiovascular screening programme from<br />
their practice<br />
• describe the PHO Per<strong>for</strong>mance Programme and its<br />
function and relevance to practice<br />
• initiate and maintain systems to collect and record<br />
in<strong>for</strong>mation identifying patients at increased risk of CVD,<br />
<strong>for</strong> example smoking, family history, ethnicity,<br />
hypertension, diabetes<br />
• manage practice team organisation and leadership to<br />
effectively deliver a CVD screening programme and risk<br />
modification intervention, and appropriate management<br />
of unstable CVD and effective secondary prevention.<br />
