Long-term Conditions<br />
Scholarship<br />
<strong>The</strong> GP will demonstrate the ability to:<br />
• undertake continuing medical education in common areas<br />
to maintain knowledge of long-term conditions<br />
• promote and facilitate education of the practice team<br />
in managing long-term conditions<br />
• audit practice management of both long-term conditions<br />
and selected screening programmes<br />
• undertake post-graduate diplomas or certificates to<br />
increase knowledge of long-term condition management.<br />
Context of <strong>General</strong> <strong>Practice</strong><br />
<strong>The</strong> GP will demonstrate the ability to:<br />
• be aware of the impact of geography and health service<br />
policies on patient care<br />
• appraise and discuss alternative therapies, including<br />
traditional therapies<br />
• familiarise themselves with key local supports at hospital<br />
and, in particular, non-governmental services, such as<br />
Diabetes <strong>New</strong> <strong>Zealand</strong><br />
• investigate local community support through allied health<br />
professionals.<br />
Management<br />
<strong>The</strong> GP will demonstrate the ability to:<br />
• develop interdisciplinary care plans with person-centred<br />
goals<br />
• familiarise themselves with key policies affecting their<br />
practice and the patient, <strong>for</strong> example using Care Plus<br />
funding <strong>for</strong> patient or practice services<br />
• be aware of practice policies on follow-up visits and<br />
charges<br />
• investigate practice activities to work towards a reduction<br />
in long-term conditions and health inequalities.<br />
• access and work with the multidisciplinary team and<br />
community services <strong>for</strong> patients with long-term conditions<br />
and disabilities<br />
