Clinical Expertise<br />

long-term conditions<br />

• familiarise themselves with and where appropriate apply<br />

Te Whare Tapa Wha and its role with the patient and<br />

their whānau<br />

• consider different aspects of managing long-term<br />

conditions with different age groups<br />

• recognise the aspects of disability that impact on<br />

everyday life<br />

• promote and enable self-management and patient<br />

responsibility<br />

• recognise the complexities of polypharmacy and aim<br />

to minimise the use of medications, as appropriate<br />

• apply their knowledge and use of screening<br />

programmes and early detection tools<br />

• recognise, understand and manage key clinical<br />

presentations.<br />

• recognise mental health aspects of long-term<br />

conditions and provide appropriate support<br />

Professionalism<br />

<strong>The</strong> GP will demonstrate the ability to:<br />

• recognise and enable patient autonomy<br />

• develop skills in maintaining self-care while managing<br />

long-term and often difficult conditions<br />

• promote self-management strategies to the patient,<br />

such as the Flinders model<br />

• engage in policy decisions to improve inter-professional<br />

communication and funding initiatives and allocations.<br />

• advocate <strong>for</strong> the patient as they negotiate the health<br />

system<br />
