and practitioners in a variety <strong>of</strong> health care disciplines (medicine, health<br />
economics, medical statistics, pharmacy, nursing and social sciences)<br />
will find it useful because health care literature is increasingly multidisciplinary<br />
and quick reference to the literature in cognate disciplines is <strong>of</strong>ten<br />
required. This extended dictionary is intended to serve as a convenient<br />
bridge.<br />
It is impossible to do justice to the many who have contributed to the<br />
literature in this area. I have cited some but hope that those I have failed to<br />
acknowledge will understand. In particular, I hope that I have not distorted<br />
the meanings <strong>of</strong> the terms they first coined. If I have done so unjustifiably<br />
in certain cases, I would be pleased to have these, and any omissions,<br />
drawn to my attention so that they can be amended in any subsequent<br />
edition.<br />
I wish to acknowledge members <strong>of</strong> my research group for sparking the<br />
idea for this book and for commenting on early drafts <strong>of</strong> the manuscript.<br />
