Chapter 10: Minimising Risk, Managing Incidents and Ensuring <strong>the</strong> Safety <strong>of</strong> <strong>the</strong> Workplace – Recommend<strong>at</strong>ions 22-31Intent <strong>of</strong> Recommend<strong>at</strong>ionThis Recommend<strong>at</strong>ion was intended to ensure th<strong>at</strong> <strong>the</strong> ADFA and ADC leadership remained aware<strong>of</strong> all incidents <strong>of</strong> concern <strong>at</strong> ADFA, and are provided with metad<strong>at</strong>a about <strong>the</strong> numbers and types <strong>of</strong>incidents occurring, so as to be able to identify and act on concerning or systemic issues.Implement<strong>at</strong>ion actionsImproved record keeping procedures have allowed for a better flow <strong>of</strong> inform<strong>at</strong>ion about incidents to <strong>the</strong>COMDT and <strong>the</strong> Director ADFA Undergradu<strong>at</strong>es (formerly <strong>the</strong> DCOMDT).A snapshot <strong>of</strong> <strong>the</strong> COMDT’s monthly return register is provided to <strong>the</strong> COMDT <strong>at</strong> monthly meetings.Australia’s Feder<strong>at</strong>ion Guard and ADFA’s postgradu<strong>at</strong>e department provide similar records using <strong>the</strong>same templ<strong>at</strong>e so th<strong>at</strong> <strong>the</strong> COMDT is aware <strong>of</strong> <strong>the</strong> any issues across all <strong>of</strong> <strong>the</strong> ADFA campus. 28 Themeetings are chaired by <strong>the</strong> COMDT, and <strong>at</strong>tended by Director ADFA Postgradu<strong>at</strong>es, Director ADFAUndergradu<strong>at</strong>es, XO Undergradu<strong>at</strong>es, <strong>the</strong> Adjutant, <strong>the</strong> Academy Legal Officer, Senior Psychologist anda Chaplain. 29Fur<strong>the</strong>r to <strong>the</strong> <strong>of</strong>ficial monthly reports th<strong>at</strong> go to <strong>the</strong> COMDT, <strong>the</strong> XO’s branch has cre<strong>at</strong>ed its ownsquadron level d<strong>at</strong>abases for management and pastoral purposes. These are examined weekly by acommittee comprised <strong>of</strong> <strong>the</strong> XO, key staff and a Chaplain. This provides a link in <strong>the</strong> chain <strong>of</strong> commandby which inform<strong>at</strong>ion can be passed up or down outside <strong>of</strong> <strong>the</strong> monthly meetings. 30The COMDT does not provide <strong>the</strong> entire report th<strong>at</strong> he receives to <strong>the</strong> Commander, ADC, but hedoes report on all <strong>at</strong>tempts <strong>at</strong> suicide, significant <strong>at</strong>tempts <strong>at</strong> self-harm, significant alleg<strong>at</strong>ions <strong>of</strong>unacceptable or criminal behaviour, and all ‘notifiable incidents.’ 31Audit findingsThe reforms to <strong>the</strong> records management systems have improved <strong>the</strong> d<strong>at</strong>a collected, and improved <strong>the</strong>processes by which this system can be reviewed by <strong>the</strong> chain <strong>of</strong> command up to <strong>the</strong> COMDT.The senior leaders <strong>of</strong> ADFA, including <strong>the</strong> COMDT and XO Undergradu<strong>at</strong>es have expressed confidencein <strong>the</strong> amount and type <strong>of</strong> inform<strong>at</strong>ion th<strong>at</strong> <strong>the</strong> new system is providing, and <strong>the</strong> ability <strong>of</strong> it to meet<strong>the</strong> requirements <strong>of</strong> <strong>the</strong> Recommend<strong>at</strong>ion. 32 The new system is in its infancy, but it appears to havestreng<strong>the</strong>ned <strong>the</strong> processes previously in place.Part <strong>of</strong> Recommend<strong>at</strong>ion 28 sought ‘timely and appropri<strong>at</strong>e’ responses to incidents, but <strong>the</strong> Auditunderstands th<strong>at</strong> <strong>the</strong> broader <strong>Defence</strong> processes can mitig<strong>at</strong>e against this. For example, during <strong>the</strong>course <strong>of</strong> <strong>the</strong> Audit, <strong>the</strong> team was made aware <strong>of</strong> certain incidents th<strong>at</strong> had been under externalinvestig<strong>at</strong>ion for months, to <strong>the</strong> frustr<strong>at</strong>ion <strong>of</strong> <strong>the</strong> undergradu<strong>at</strong>es concerned. 33 There is some evidenceth<strong>at</strong> <strong>the</strong> length <strong>of</strong> time external investig<strong>at</strong>ions take is a source <strong>of</strong> frustr<strong>at</strong>ion throughout <strong>Defence</strong>, and oneabout which individual units or bases can do little without more systemic reform. 34 ADFA is <strong>at</strong>tempting tomanage <strong>the</strong>se frustr<strong>at</strong>ions through its squadron level tracking, and provide feedback to those concernedas investig<strong>at</strong>ions continue.112
Recommend<strong>at</strong>ion 29: In order th<strong>at</strong> standards <strong>of</strong> reporting, recordingand resolving incidents are properly met, ADFA should ensure <strong>the</strong>d<strong>at</strong>abase undergoes annual quality assurance testing to determine:a) whe<strong>the</strong>r all complaints and incidents are being entered on<strong>the</strong> d<strong>at</strong>abase and all required fields in <strong>the</strong> d<strong>at</strong>abase areadequ<strong>at</strong>ely completedb) whe<strong>the</strong>r <strong>the</strong> record keeping and reporting standards in <strong>the</strong>Management and Reporting <strong>of</strong> Unacceptable Behaviour,Management and Reporting <strong>of</strong> Sexual Offences (includingForms AC 875-1 – AC 875-3) and Quick AssessmentInstructions are being met in rel<strong>at</strong>ion to all individualcomplaints <strong>of</strong> unacceptable behaviour or sexual <strong>of</strong>fences.Intent <strong>of</strong> Recommend<strong>at</strong>ionThe aim <strong>of</strong> this Recommend<strong>at</strong>ion was to ensure th<strong>at</strong> <strong>the</strong> correct processes were followed in compilingd<strong>at</strong>abases as a means <strong>of</strong> quality control.Implement<strong>at</strong>ion actionsThe new incidents register was only months old <strong>at</strong> <strong>the</strong> completion <strong>of</strong> <strong>the</strong> Audit, and so annual qualityassurance testing has not yet been undertaken. Discussions with staff indic<strong>at</strong>e a belief th<strong>at</strong> <strong>the</strong> broaderreforms to <strong>the</strong> reporting and d<strong>at</strong>abase management systems include processes th<strong>at</strong> ADFA considers willmeet <strong>the</strong> intent <strong>of</strong> this Recommend<strong>at</strong>ion.Audit findingsADFA’s new d<strong>at</strong>abase management process has provided some level <strong>of</strong> quality assurance, but <strong>the</strong>re isnot a formal annual quality assurance testing process in place yet.The IGADF military justice audit will continue to be conducted, but this is not an annual process. TheAudit team understands th<strong>at</strong> ADFA is currently investig<strong>at</strong>ing ano<strong>the</strong>r mechanism to conduct internalaudits on <strong>the</strong>ir administr<strong>at</strong>ion systems but th<strong>at</strong> this is not in place yet. 35While <strong>the</strong>re is not a formal annual test in place, <strong>the</strong> way th<strong>at</strong> <strong>the</strong> new system has been designedprovides for some internal checking mechanisms. For example, individuals completing a QA are givena folder with directives and guidance th<strong>at</strong> assists in <strong>the</strong> QA process, and completed QAs are checkedby <strong>the</strong> Adjutant when entering <strong>the</strong>m on <strong>the</strong> QA register.
