Recommend<strong>at</strong>ion 29: In order th<strong>at</strong> standards <strong>of</strong> reporting, recordingand resolving incidents are properly met, ADFA should ensure <strong>the</strong>d<strong>at</strong>abase undergoes annual quality assurance testing to determine:a) whe<strong>the</strong>r all complaints and incidents are being entered on<strong>the</strong> d<strong>at</strong>abase and all required fields in <strong>the</strong> d<strong>at</strong>abase areadequ<strong>at</strong>ely completedb) whe<strong>the</strong>r <strong>the</strong> record keeping and reporting standards in <strong>the</strong>Management and Reporting <strong>of</strong> Unacceptable Behaviour,Management and Reporting <strong>of</strong> Sexual Offences (includingForms AC 875-1 – AC 875-3) and Quick AssessmentInstructions are being met in rel<strong>at</strong>ion to all individualcomplaints <strong>of</strong> unacceptable behaviour or sexual <strong>of</strong>fences.Intent <strong>of</strong> Recommend<strong>at</strong>ionThe aim <strong>of</strong> this Recommend<strong>at</strong>ion was to ensure th<strong>at</strong> <strong>the</strong> correct processes were followed in compilingd<strong>at</strong>abases as a means <strong>of</strong> quality control.Implement<strong>at</strong>ion actionsThe new incidents register was only months old <strong>at</strong> <strong>the</strong> completion <strong>of</strong> <strong>the</strong> Audit, and so annual qualityassurance testing has not yet been undertaken. Discussions with staff indic<strong>at</strong>e a belief th<strong>at</strong> <strong>the</strong> broaderreforms to <strong>the</strong> reporting and d<strong>at</strong>abase management systems include processes th<strong>at</strong> ADFA considers willmeet <strong>the</strong> intent <strong>of</strong> this Recommend<strong>at</strong>ion.Audit findingsADFA’s new d<strong>at</strong>abase management process has provided some level <strong>of</strong> quality assurance, but <strong>the</strong>re isnot a formal annual quality assurance testing process in place yet.The IGADF military justice audit will continue to be conducted, but this is not an annual process. TheAudit team understands th<strong>at</strong> ADFA is currently investig<strong>at</strong>ing ano<strong>the</strong>r mechanism to conduct internalaudits on <strong>the</strong>ir administr<strong>at</strong>ion systems but th<strong>at</strong> this is not in place yet. 35While <strong>the</strong>re is not a formal annual test in place, <strong>the</strong> way th<strong>at</strong> <strong>the</strong> new system has been designedprovides for some internal checking mechanisms. For example, individuals completing a QA are givena folder with directives and guidance th<strong>at</strong> assists in <strong>the</strong> QA process, and completed QAs are checkedby <strong>the</strong> Adjutant when entering <strong>the</strong>m on <strong>the</strong> QA register.
