DYSPEPSIA<br />

Indigestion may be caused <strong>by</strong> org<strong>an</strong>ic disease in the<br />

gastrointestinal tract <strong>an</strong>d <strong>by</strong> m<strong>an</strong>y diseases originating<br />

elsewhere. Consideration will be given here mainly to symptoms<br />

that occur in the absence <strong>of</strong> demonstrable org<strong>an</strong>ic disease.<br />

Common causes are eating too much or too<br />

rapidly,<br />

inadequate mastication (frequently due to poor dentition)<br />

eating during emotional upsets or severe mental strain, <strong>an</strong>d<br />

swallowing large amounts <strong>of</strong> air. Other factors are excessive<br />

smoking; constipation; <strong>an</strong>d ingestion <strong>of</strong> poorly cooked<br />

foods,<br />

those with high fat content <strong>an</strong>d others such as cucumbers,<br />

radishes<br />

<strong>an</strong>d gas-forming vegetables C e »9* be<strong>an</strong>s, cabbage, turnips, onions).<br />

Most <strong>of</strong> the symptoms result from altered<br />

motor activity. Fats inhibit such activity, decrease<br />

gastric<br />

peristalsis,<br />

lower gastric tone <strong>an</strong>d prolong gastric emptying time. Moderate<br />

distension <strong>of</strong> the stomach stimulates motility, while<br />

distension, such as occurs from overeating., inhibits<br />

marked<br />

motility<br />

<strong>an</strong>d may produce a sensation <strong>of</strong> epigastric distension <strong>an</strong>d<br />

fullness<br />

<strong>an</strong>d nausea. Nervousness <strong>an</strong>d <strong>an</strong>xiety tend to increase peristalsis,<br />

while fear, shock, depression, pain <strong>an</strong>d physical fatigue tend<br />

to inhibit it <strong>an</strong>d reduce gastric<br />

tone.
