2. Histamine reactions: headache (55), flushing (43), nausea and<br />

vomiting (1), fall in blood pressure (1)<br />

3. Neurological Disturbances: vertigo (33), tinnitus (13), deafness (6)<br />

4. Local irritation: pain, soreness and induration (15) tingling (2) and<br />

parathesias (3)<br />

5. Miscellaneous problems: diarrhoea (1), albuminuria and casts (2),<br />

purpura haemorrhagica (1) and arthralgia (1)<br />

6. Urine: nine out of ten had hyaline and granular casts<br />

Information given on time to onset and duration of ADRs. Overall<br />

20.5% of patients had adverse reactions and at 3G daily 46% had<br />

reactions, whilst at ≥ 4 G 60% had reactions. When the average<br />

daily dose exceeded 1 Gm there was an increase in the number<br />

of ADRs. (Keefer et al., 1946).<br />

One hundred and four patients. No individual ADR mentioned. Dose<br />

between 1 and 3G daily<br />

General mention: nausea, malaise, fever, renal irritation,<br />

arthralgia, local reactions at site of injection (possibly due to<br />

impurities), tinnitus and dizziness, and skin reactions (urticaria,<br />

maculopapular or generalized dermatitis) (Nichols & Herrell, 1946).<br />

One hundred patients. Dose 1–3 gm daily. No individual ADRs.<br />

1. Vestibular damage with frequent persistence of diminished<br />

response to caloric stimulation for at least several weeks after<br />

dechallenge.<br />

2. Irritation at site of injection and is inversely proportional to the<br />

purity of the product.<br />

3. Histamine-like reactions caused by impurities (Hinshaw et al.,<br />

1946).<br />

Otic Complications. One hundred and sixty one patients. Dose 3 gm<br />

daily. There was definite toxicity in 3% of patients.<br />

1. Hearing problems: 65% had high tone loss and 13.6 % of these<br />

also had low tone loss whilst 32.1% had normal hearing.<br />

2. Vestibular function: caloric stimulation showed a normal response<br />

in 80.2% of patients (Fowler & Seligman, 1947).<br />

1947 June 21 st Thirty one patients received 3 gm of highly purified streptomycin daily
