active, you must continue filing reports <strong>for</strong><br />

6.6 Requirements <strong>for</strong> an Application <strong>for</strong> a Long-Term <strong>Gas</strong> Removal Permit<br />

1) Long-Term <strong>Gas</strong> Removal Application <strong>for</strong>m The Long-Term <strong>Gas</strong> Removal Application<br />

<strong>for</strong>m shown in Appendix G must be used to<br />

request a new gas removal permit or an<br />

amendment to an existing gas removal permit.<br />

The ERCB will accept an application only if it<br />

is filed using the <strong>for</strong>m.<br />

An applicant is not required to give notification<br />

to any party or advertise its intent to apply <strong>for</strong> a<br />

long-term gas removal permit or <strong>for</strong> an<br />

amendment to an existing permit.<br />

2) Change of permit holder To request any change in the named permit<br />

holder, check the box in front of the words<br />

“Change of permit holder” <strong>and</strong> the box in front<br />

of the statement beginning “The proposed<br />

permit holder….”<br />

It is very important to check this agreement<br />

box, as this confirms that the proposed permit<br />

holder is fully prepared to take on the<br />

obligations of the previous permit holder with<br />

respect to the permit. The ERCB will not<br />

process a request to change the name on a<br />

permit unless this box has been checked.<br />

3) Permit(s) No. to be rescinded Complete this portion of the <strong>for</strong>m only when<br />

you want to rescind an existing permit;<br />

otherwise, leave it blank.<br />

4a) Total volume of gas proposed <strong>for</strong> removal If you wish to amend an existing long-term gas<br />

removal permit to add a new market with an<br />

associated new volume, fill out the “Existing<br />

volume authorized” line to reflect the volume<br />

currently authorized <strong>for</strong> removal under the<br />
