a) the point at which the common purchaser<br />

shall take delivery of gas offered <strong>for</strong> sale,<br />

<strong>and</strong>/or<br />

b) the proportion of gas that the common the<br />

common purchaser shall purchase from<br />

each producer or owner offering gas <strong>for</strong><br />

sale, <strong>and</strong>/or<br />

c) the price that the common purchaser shall<br />

pay <strong>for</strong> the gas.<br />

3) A discussion of the reasons why you are<br />

requesting the subject order, including a<br />

description of the circumstances that in your<br />

opinion have led or will lead to an inequitable<br />

situation.<br />

4) Evidence that your well is completed in the<br />

pool to which the common purchaser is to<br />

apply, including<br />

a) a net pay isopach map of the pool,<br />

including both oil <strong>and</strong> gas if there is an<br />

associated pool,<br />

The net pay map should show the entire pool<br />

<strong>and</strong> illustrate wells outside the pool that control<br />

the edges of the pool.<br />

b) where pool delineation or fluid interfaces<br />

are based on structural interpretation, a<br />

structure contour map of the pool <strong>and</strong><br />

offsetting area,<br />

c) an interpreted <strong>and</strong> annotated log crosssection<br />

or representative well log(s)<br />

showing<br />

i) stratigraphic interpretation of the<br />

zone(s) of interest,<br />

If there is a dispute among the parties involved<br />

regarding the delineation of the pool(s) in<br />

question, you should include an annotated<br />

cross-section in the application; if there is no<br />

dispute respecting this issue, an annotated<br />

representative well log is sufficient.<br />

ii) interpretation of the fluid interfaces<br />

present,<br />

iii) completions <strong>and</strong> treatments to the<br />

wellbore(s), with dates,<br />

iv) cumulative production,<br />

v) finished drilling date <strong>and</strong> kelly bushing<br />

(KB) elevation, <strong>and</strong><br />
