7) Are there outst<strong>and</strong>ing concerns from well licensees? If yes, the Licensee Concerns<br />

attachment must be submitted as part of the application, in accordance with <strong>Directive</strong><br />

<strong>065</strong>.<br />

YES means that there are unresolved concerns or objections from one or more of the well<br />

licensees that were notified of the application. The in<strong>for</strong>mation required in a Licensee<br />

Concerns attachment is identified in Section 2.1.3.3(C) of this directive.<br />

NO means that there are no known unresolved well licensee concerns from any of the well<br />

licensees that were notified of the application.<br />

Proposed Injection Well Locations <strong>and</strong> Injection Intervals<br />

This section must only be completed <strong>for</strong> new ER schemes <strong>and</strong> amendment types requesting to<br />

add injection well location(s).<br />

8) An ER scheme application cannot be submitted unless the proposed injection wells have<br />

been drilled.<br />

Have the proposed injection wells been drilled?<br />

YES means that all the proposed injection wells have been drilled.<br />

NO means that all the proposed injection wells have not been drilled. The ERCB is not<br />

prepared to accept an ER application when the proposed injectors are not drilled because of<br />

the potential this creates <strong>for</strong> changes to the scheme details (commencement of injection date,<br />

bottomhole location of injector, <strong>and</strong> the unconfirmed presence <strong>and</strong> quality of the reservoir).<br />

9) An ER scheme application cannot be submitted unless the source of the proposed<br />

injection fluid has been secured.<br />

Has the source of the proposed injection fluid been secured?<br />

YES means that the supply of injection fluid is secured <strong>and</strong> will be available <strong>for</strong> use by the<br />

proposed injection date.<br />

If nonsaline water is to be used <strong>for</strong> injection, you must have a valid water diversion permit<br />

from Alberta Environment. The water diversion permit number must be provided in the<br />

Application attachment.<br />

NO means that the injection fluid source has not been secured. The ERCB is not prepared to<br />

accept an ER application when the injection fluid has not been secured because of the<br />

potential <strong>for</strong> delays in the commencement of injection.<br />

10) Provide the following <strong>for</strong> the proposed injection well locations:<br />

Well Licence Number: The well licence number issued by the ERCB <strong>for</strong> the proposed<br />

injection well.<br />

Unique Well Identifier (UWI): The UWI (LE/LSD-SEC-TWP-RGEWMER/ES) associated<br />

with the Well Licence Number. The full unique well identifier is listed in the approval; it is<br />
