conclusions reached by tedious repetitions and platitudes.<br />

“On closer exam<strong>in</strong>ation, it appears that this entire body of work is largely self‐referential.<br />

“It appeals for validation to itself and all is framed with<strong>in</strong> the same policy agenda.<br />

“The Scientific and Conceptual Basis of Incapacity Benefits (authors Gordon Waddell and Mansel Aylward) is<br />

particularly <strong>in</strong>terest<strong>in</strong>g <strong>in</strong> this connection (and) it is useful for reveal<strong>in</strong>g the th<strong>in</strong>k<strong>in</strong>g beh<strong>in</strong>d the Green Paper. Its<br />

source is the UNUMProvident Centre for Psychosocial and Disability Research located <strong>in</strong> Cardiff University s<strong>in</strong>ce<br />

2004. (Aylward’s) remit is ‘to develop specific l<strong>in</strong>es of research <strong>in</strong>to the psychosocial factors related to disability,<br />

vocational rehabilitation, and the ill‐health behaviours which impact on work and employment’.<br />

“UNUMProvident is the largest disability <strong>in</strong>surance company <strong>in</strong> the world and is <strong>in</strong>volved <strong>in</strong> a number of lawsuits for<br />

‘bad faith’ <strong>in</strong> refus<strong>in</strong>g to honour disability <strong>in</strong>surance claims.<br />

“This re<strong>in</strong>forces the caution aga<strong>in</strong>st tak<strong>in</strong>g this apparently impressive body of work at face value. It is not<br />

research undertaken <strong>in</strong> the spirit of open enquiry. It is commissioned research and, as such, pre‐disposed<br />

towards ideologically determ<strong>in</strong>ed outcomes.<br />

“GPs, who first certify claimants, do not ‘understand the importance of work for health’. This reviews different ways of<br />

bypass<strong>in</strong>g them, <strong>in</strong>clud<strong>in</strong>g access to confidential GP records…. ‘For common health problems, the doctors’ op<strong>in</strong>ion…is<br />

unfounded, of limited value and can be counter‐productive’ (The Scientific and Conceptual Basis of Incapacity Benefits,<br />

p 145‐146)”.<br />

Comment<strong>in</strong>g on the political philosophy underp<strong>in</strong>n<strong>in</strong>g this “reform”, Ravetz says:<br />

“The broad context is the Government’s ‘new vision’ of a reformed welfare state, where relations between state and<br />

citizen constitute a ‘contract’ <strong>in</strong> which rights of the citizen are balanced by obligations (and) the sick and disabled are<br />

not exempt from this contract.<br />

“Under the ‘contract’, the obligations of claimants are to ‘recognise that symptoms, feel<strong>in</strong>g unwell, sickness and<br />

<strong>in</strong>capacity are not the same’. To this is added: ‘The greater the subjectivity and personal / psychological elements <strong>in</strong><br />

<strong>in</strong>capacity, the greater the degree of personal responsibility’. Should they fail to carry out their obligations, claimants<br />

must be subject to sanctions.<br />

“The assumptions about illness/disability made (by Waddell and Aylward) and <strong>in</strong> the Green Paper must give rise to<br />

concerns. The whole emphasis is on de‐coupl<strong>in</strong>g health problems and medical conditions from unfitness for work.<br />

“We are expected to be impressed by the unacceptable and spirall<strong>in</strong>g numbers of claimants…but we are not presented<br />

(as would be the case <strong>in</strong> genu<strong>in</strong>ely objective research) with the data from which to evaluate the conclusions drawn.<br />

“Claimants are depicted as a drag on the economy and money spent on them as good as poured <strong>in</strong>to a black hole.<br />

“The underly<strong>in</strong>g philosophy belittles sick and disabled people. By imply<strong>in</strong>g that they are parasites, it excludes them<br />

more <strong>in</strong>sidiously from the body politic than the system it seeks to replace.<br />

“(The) Case Management Programmes (CMPs) are based ma<strong>in</strong>ly on cognitive behavioural therapy.<br />

“Another crux is rely<strong>in</strong>g on CMPs as a tool for coax<strong>in</strong>g people out of patterns of ‘illness behaviour’. They are applied<br />

by occupational and physio‐ therapists, who lack expert knowledge of the diseases clients may have. They make much<br />

use of watered‐down cognitive behavioural therapy which, delivered <strong>in</strong>expertly and <strong>in</strong> group situations, can add to the
