a functional somatic syndrome), a condition associated with gross bladder wall changes, and pa<strong>in</strong>ful<br />

bladder syndrome, which exhibits the same symptoms but lacks gross cystoscopic f<strong>in</strong>d<strong>in</strong>gs (Serge<br />

Mar<strong>in</strong>kovic et al; BMJ 8 th August 2009:339:337‐342). The authors stated that patients with IC are 100 times<br />

more likely to have irritable bowel syndrome and are 30 times more likely to have systemic lupus<br />

erythematosus, and that other associated chronic illnesses <strong>in</strong>clude fibromyalgia and chronic fatigue<br />

syndrome. The authors provided a compell<strong>in</strong>g but unconfirmed theory – based on evidence that the<br />

authors say represents the majority op<strong>in</strong>ion of researchers actively <strong>in</strong>volved <strong>in</strong> the field – of likely<br />

autoimmune causation:<br />

“The pathological features of bladder epithelial damage and related blood vessel transitions <strong>in</strong> the absence of <strong>in</strong>fection<br />

have been recognised for more than 100 years… One theory is that <strong>in</strong>creased permeability of the protective<br />

glycosam<strong>in</strong>oglycan l<strong>in</strong><strong>in</strong>g of the bladder epithelium causes potassium (and) tox<strong>in</strong>s to leak <strong>in</strong>to the mucosal<br />

<strong>in</strong>terstitium, activat<strong>in</strong>g mast cells and generat<strong>in</strong>g an autoimmune response. Mast cells produce immune reactive<br />

chemicals, which <strong>in</strong> turn cause generalised bladder <strong>in</strong>flammation and bladder mucosal damage through the presence of<br />

tachyk<strong>in</strong><strong>in</strong>s and cytok<strong>in</strong>es. These further mediate the release of histam<strong>in</strong>e, tumour necrosis factor, chymase, tryptase,<br />

and prostagland<strong>in</strong>s. F<strong>in</strong>ally, <strong>in</strong>flammatory agents sensitise bladder neurones, produc<strong>in</strong>g pelvic and bladder<br />

pa<strong>in</strong>…..Some patients have exacerbations of their symptoms after <strong>in</strong>gest<strong>in</strong>g certa<strong>in</strong> food or dr<strong>in</strong>ks….Urothelial cell<br />

cultures express abnormal gene variants. When urothelial biopsies…were subjected to stretch…they released<br />

significantly higher concentrations of ATP than control biopsies, suggest<strong>in</strong>g that ATP plays an important role <strong>in</strong> this<br />

syndrome. An <strong>in</strong>vestigation of cultured bladder urothelial cells…showed that such cells had higher than normal<br />

concentrations of ATP, which decreases the ability of the bladder wall to conduct potassium ions…which aga<strong>in</strong><br />

<strong>in</strong>dicates that impaired potassium conduction is <strong>in</strong>volved <strong>in</strong> the pathophysiology of <strong>in</strong>terstitial cystitis”.<br />

Wessely, however, seemed immediately to reject outright any autoimmune or allergic component: “The<br />

article…details associations with fibromyalgia, chronic fatigue syndrome and, strik<strong>in</strong>gly, a 100‐fold <strong>in</strong>creased risk of<br />

irritable bowel syndrome – all of which have good evidence for the role, at least <strong>in</strong> part, of psychological factors <strong>in</strong> the<br />

their aetiology or ma<strong>in</strong>tenance…It is highly possible that psychological factors have an aetiological contribution to<br />

conditions such as pa<strong>in</strong>ful bladder syndrome. Such disorders, where physical pathology cannot fully account for<br />

symptoms, are known as ‘medically unexpla<strong>in</strong>ed’ or ‘functional’ (somatic) syndromes…It has been proposed (cit<strong>in</strong>g<br />

his own Lancet paper 1999:354:936‐939) that they may be the same underly<strong>in</strong>g disorder manifest<strong>in</strong>g itself <strong>in</strong><br />

different bodily systems…Dr Mar<strong>in</strong>kovic, however, despite draw<strong>in</strong>g out the evidence for such a description, seems to<br />

resist the <strong>in</strong>ference, mak<strong>in</strong>g no mention of psychological factors even as possible contributors to the aetiology…The<br />

experience of other functional somatic syndromes…is that a biopsychosocial approach is the foundation of successful<br />

cognitive behavioural therapy. This…surely deserves a place <strong>in</strong> any review (of) pa<strong>in</strong>ful bladder syndrome”<br />

People must decide for themselves whether or not, based on the evidence, Dr Mar<strong>in</strong>kovic did “draw out the<br />

evidence” that IC is a functional somatic disorder, and which of the two theories they believe.<br />

Professor Nancy Klimas from the University of Miami has confirmed that <strong>in</strong>terstitial cystitis overlaps with<br />

<strong>ME</strong>/CFS (New York Times, 21 st January 2010: http://consults.blogs.nytimes.com/2010/01/21/hiv‐<br />

fibromyalgia‐and‐chronic‐fatigue‐syndrome/ ).<br />

Based on the evidence, a m<strong>in</strong>iscule amount of which has been <strong>in</strong>cluded <strong>in</strong> this Report, people must also<br />

decide for themselves whether the Wessely School is correct that <strong>ME</strong>/CFS is a behavioural disorder that will<br />

respond favourably to their cognitive restructur<strong>in</strong>g <strong>in</strong>terventions that are be<strong>in</strong>g studied <strong>in</strong> the MRC PACE<br />

Trial.<br />

which follow may help them make up their m<strong>in</strong>d.
