is a very good person to talk to in order to review these things. We'd really like you to make an<br />

appointment with your family <strong>do</strong>ctor in the next few days. Could you <strong>do</strong> that? In the meantime, if<br />

you're feeling really unsafe, you can always take yourself to the local emergency department or<br />

ask a family member or friend to take you. The crisis line is also a great resource to have. They<br />

can help you in a crisis situation or can help you access resources such as your local emergency<br />

department. The emergency address in this area and the crisis number is in your resource<br />

pamphlet. Do you need another one? (if Yes)We can e-mail it or fax it to you.<br />

3. If the answer to Q 2 was:<br />

V^NO: No referral to school counsellor is made unless respondent scores<br />

above the cut <strong>of</strong>f score for potential <strong>depression</strong> in the RADS-2 score, or the critical items<br />

in the scale add up to three (includes Item #14) or more items scored in this measure or<br />

both critical items add up to three or more items scored and the respondent total score is<br />

within the score for potential <strong>depression</strong> and needs further assessment. No contact with<br />

the respondent's parent/Guardian is made.<br />

V^YES: The school counsellor will be provided the name <strong>of</strong> the participant and<br />

will be informed that we have recommended this participant make contact with the school<br />

counsellor. Also, mention to the school counsellor that we will also contact the respondent's<br />

parent/guardian, and that we are recommending that their child check in with the school<br />

counsellor for further follow up. The RADS-2 are scored at school immediately after completion<br />

<strong>of</strong> the Questionnaire.<br />

Those respondents who answer 3 or 4 on Question #14, will be contacted in school if the school<br />

is in session. Arrangements will be made with the school counsellor in <strong>how</strong> best to contact these<br />

respondents during school hours. If the Questionnaire is completed just before school classes end<br />

for the day, the respondent will be contacted at home that same evening. For those respondents<br />

who answer YES to Q2 during the evening, the parent or guardian will be contacted within 2<br />

hours by telephone. The respondent will be referred to the school counsellor the next morning. If<br />

the respondent answers YES to Q2 during school hours, the school counsellor will be notified<br />

within 2 hours by face contact or telephone. The parent or guardian will be notified by telephone<br />

that day or evening on the same day <strong>of</strong> interview.<br />

4. Prepare a written report file that includes the name <strong>of</strong> the school counsellor who<br />

received the information, date and time <strong>of</strong> the referral to the school counsellor, if the<br />

parent/guardian was contacted. No identifiable information is included in the report file.<br />

RADS-2 raw score range 76-120 or >61T on the Depression Total Scale, or a score <strong>of</strong> 4 on<br />

Critical Items 3, 6, 19, 20, and 30 on the RADS-2, or a score <strong>of</strong> 3 or more on the Critical<br />

Items that include Critical Item 14 (self-destructive behavior), or a combination <strong>of</strong> a raw<br />

score range <strong>of</strong> 76-120 (or 61T) on the Depression Total Scale and a score <strong>of</strong> 4 on Critical<br />

Items 3, 6, 19, 20, and 30 or 3 or more Critical Items that also include Item 14<br />
