SAMPLE CONTRACT<br />

10.13 The CSWs shall obtain parental or Juvenile Court consent, as needed, for<br />

the Placed Child’s medical <strong>and</strong> dental care, mental health treatment, <strong>and</strong><br />

participation in recreational <strong>and</strong> school activities.<br />

10.14 CSW shall provide CONTRACTOR with a copy <strong>of</strong> the court authorization<br />

for psychotropic medication, when applicable, within one day <strong>of</strong><br />

placement.<br />

11.0 DESCRIPTION OF SERVICES<br />

11.1 CONTRACTOR covenants <strong>and</strong> agrees to provide all Services as described<br />

in this Contract <strong>and</strong> set forth in the Statement <strong>of</strong> Work (Exhibit A) <strong>of</strong> this<br />

Contract. CONTRACTOR shall provide such Services to each Placed<br />

Child in accordance with CONTRACTOR'S Program Statement (Exhibit B).<br />

CONTRACTOR warrants that it possesses the competence, expertise <strong>and</strong><br />

personnel necessary to provide such Services.<br />

11.2 CONTRACTOR has submitted a Program Statement to COUNTY in<br />

accordance with the Program Statement Guidelines <strong>of</strong> CCLD.<br />

CONTRACTOR's Program Statement will include, but not be limited to,<br />

specific statements defining intake policy, treatment Services <strong>and</strong> policies,<br />

replacement <strong>and</strong> discharge policies, detailed statements <strong>of</strong> the total<br />

Services provided by CONTRACTOR, staffing, <strong>and</strong> the expenditure<br />

statement submitted to the rate setting <strong>and</strong> licensing agencies.<br />

CONTRACTOR's performance under this Contract will be evaluated in part<br />

based on CONTRACTOR's Program Statement.<br />

11.3 COUNTY may, during the term <strong>of</strong> this Contract, request that<br />

CONTRACTOR make revisions to its Program Statement by notifying<br />

CONTRACTOR in writing thirty (30) days in advance <strong>of</strong> any proposed<br />

changes. Also, CONTRACTOR shall submit a revised Program Statement<br />

to COUNTY at any time during the term <strong>of</strong> this Contract when<br />

CONTRACTOR makes changes to the program. COUNTY shall review<br />

such Program Statement revisions for approval, <strong>and</strong> once accepted by<br />

COUNTY, written approval shall be provided to CONTRACTOR.<br />

CONTRACTOR’s revised Program Statement shall become a part <strong>of</strong> this<br />

Contract as Exhibit B in accordance with Part II, Section 5.0, Changes <strong>and</strong><br />

Amendments.<br />

12.0 CERTIFIED FOSTER PARENTS<br />

12.1 CONTRACTOR agrees to recruit, certify, train, monitor <strong>and</strong> provide<br />

pr<strong>of</strong>essional support to Certified Foster Parents in compliance with<br />

California Code <strong>of</strong> Regulations, <strong>and</strong> this Contract, including any future<br />

amendments thereto.<br />
