1.1.7 Required Services by CONTRACTOR’S Social Work Staff<br />

EXHIBIT A<br />

The CONTRACTOR’S social work staff shall provide Services as set forth in<br />

the CONTRACTOR’S Program Statement, Exhibit B <strong>of</strong> the Contract, <strong>and</strong> in<br />

Title 22, Division 6, Chapter 8.8, Sections 88065(a)(9), 88065.3(a) <strong>and</strong> (g),<br />

88068.1(b)(2) <strong>and</strong> (3), 88068.2(a), <strong>and</strong> 88068.3(a). These Services shall<br />

include: (1) qualified social work personnel available on a 24-hour basis to<br />

respond to any emergency within a two-hour period; (2) provision <strong>of</strong> social<br />

Services <strong>of</strong> the FFA; (3) orientations for potential Certified Foster Parents;<br />

(4) initial <strong>and</strong> continued evaluation <strong>and</strong> assessment <strong>of</strong> Certified Foster<br />

Parents <strong>and</strong> Certified Foster Parents’ homes; (5) intake <strong>and</strong> continued<br />

assessments <strong>of</strong> Placed <strong>Children</strong> to determine if the FFA can meet the<br />

Placed Child’s Service needs; (6) placement <strong>of</strong> the child in the Certified<br />

Family Home; (7) development <strong>of</strong> the Needs <strong>and</strong> Services Plan <strong>and</strong> updates<br />

<strong>of</strong> the Needs <strong>and</strong> Services Plan; (8) supervision <strong>of</strong> the placement including<br />

direct contact with the Placed Child <strong>and</strong> the Certified Foster Parents; <strong>and</strong> (9)<br />

provision <strong>of</strong> support Services to the Certified Foster Parents.<br />

1.1.8 Required Visits by Social Work Staff with Placed Child <strong>and</strong> Certified<br />

Foster Parents<br />

The CONTRACTOR’S social work staff shall make weekly face-to-face<br />

contacts with the Placed Child during the first three months <strong>of</strong> placement.<br />

Two <strong>of</strong> the weekly contacts each month shall occur in the Certified Family<br />

Home. These requirements apply to: (1) initial placements <strong>of</strong> children <strong>of</strong><br />

any age; (2) any subsequent replacements within the FFA <strong>of</strong> children five<br />

years old <strong>and</strong> younger for three months <strong>and</strong> <strong>of</strong> children six years <strong>and</strong> older<br />

for one month. Thereafter, there shall be no fewer than two face-to-face<br />

contacts per month, approximately 14 Days apart, with each Placed Child<br />

<strong>and</strong> Certified Foster Parent. The FFA social workers shall interview the<br />

Placed <strong>Children</strong> privately at least monthly regarding quality <strong>of</strong> life issues<br />

<strong>and</strong> shall document the results <strong>of</strong> these interviews.<br />

The CONTRACTOR’S social work staff shall document each visit with the<br />

Placed Child <strong>and</strong> the Certified Foster Parents <strong>and</strong> include the following<br />

information at minimum: (1) name or initials <strong>of</strong> the person recording; (2)<br />

both the date recorded <strong>and</strong> the date <strong>of</strong> the visit; (3) problems reported by<br />

the child; (4) problems reported by the Certified Foster Parents; <strong>and</strong> (5)<br />

how the reported problems are being resolved.<br />

1.2 Monitoring Requirements:<br />

1.2.1 Requirements <strong>and</strong> Duties <strong>of</strong> Certified Foster Parents <strong>and</strong> Others Who<br />

Supervise Placed <strong>Children</strong><br />

The CONTRACTOR shall Monitor for Compliance that the Certified Foster<br />

Parents <strong>and</strong> others who supervise the Placed <strong>Children</strong> meet all the<br />
