for the Placed Child to receive monthly evaluations by the prescribing<br />

physician unless otherwise documented by the physician.<br />

For each psychotropic medication prescribed to a Placed Child, the<br />

CONTRACTOR, in conjunction with the CSW, shall monitor to ensure that:<br />

(1) the prescribing physician submits a request <strong>and</strong> obtains court<br />

authorization; <strong>and</strong> (2) these requests <strong>and</strong> orders are renewed every six<br />

(6) months (Exhibit A-X). Upon receipt from the CSW or physician, the<br />

CONTRACTOR shall maintain copies <strong>of</strong> the court authorizations in the<br />

Placed Child’s case record.<br />

The CONTRACTOR shall monitor the incorporation <strong>of</strong> all psychotropic<br />

medication(s) the Placed Child receives into the treatment plan.<br />

3.9.5 Plans for Emergency Medical <strong>and</strong> Dental Treatment<br />

The CONTRACTOR shall Monitor for Compliance that Certified Foster<br />

Parents have plans for emergency medical <strong>and</strong> dental treatment <strong>of</strong> a<br />

Placed Child.<br />

3.9.6 Grooming <strong>and</strong> Hygiene<br />

The CONTRACTOR shall Monitor for Compliance that Certified Foster<br />

Parents provide Placed <strong>Children</strong> age-appropriate instructions in proper<br />

grooming <strong>and</strong> personal hygiene.<br />

3.9.7 Secure Location for Records<br />

The CONTRACTOR shall Monitor for Compliance that Certified Foster<br />

Parents maintain in a secure location (inaccessible to children) all<br />

documents set forth in Title 22, Division 6, Chapter 4, Section 83<strong>07</strong>0.<br />

3.10 Educational Requirements:<br />

3.10.1 Stable School Placements<br />

The CONTRACTOR shall comply with WIC Section 16000(b).<br />

CONTRACTOR shall also comply with Education Code Section 48850(a),<br />

which states, in part, that, “In fulfilling their responsibilities to pupils in<br />

foster care, educators, COUNTY placing agencies, care providers,<br />

advocates, <strong>and</strong> the juvenile courts shall work together to maintain stable<br />

school placements <strong>and</strong> to ensure that each pupil is placed in the least<br />

restrictive educational programs, <strong>and</strong> has access to the academic<br />

resources, services, <strong>and</strong> extracurricular <strong>and</strong> enrichment activities that are<br />

available to all pupils. In all instances, educational <strong>and</strong> school placement<br />

decisions must be based on the best interests <strong>of</strong> the child.”<br />

3.10.2 Right <strong>of</strong> Placed Child to Remain in School <strong>of</strong> Origin<br />
