5. Discuss <strong>and</strong> obtain SCSW written approval for the release <strong>of</strong> the required records.<br />

Document SCSW's approval in the Case Notes section in CWS/<strong>CMS</strong>. Print a hard<br />

copy <strong>of</strong> the documentation <strong>and</strong> give it to the SCSW for signature. File it in the<br />

Additional Services Documentation Folder.<br />

6. Photocopy the requested records. Review carefully, black-out any unauthorized<br />

information <strong>and</strong> photocopy the initial copy. Release the second copy <strong>and</strong> ensure<br />

that the initial altered copy is shredded.<br />

D. WHEN: SCHOOLS REQUEST RECORDS<br />

The school system is expected to obtain the child’s school records from the previous<br />

school. If for some reason the school records (including immunization records) are<br />

unavailable, the caregiver may release only those records necessary to enroll the child<br />

in school. No other records/documents shall be released to the school.<br />

E. WHEN: A PRIVATE ADOPTION AGENCIES PERFORMING<br />

ADOPTION HOME STUDIES REQUEST RECORDS<br />

For purposes <strong>of</strong> completing adoption home studies the <strong>Department</strong> shall utilize only<br />

adoption agencies that are licensed by the state in which they provide services.<br />

All identifying information regarding the birth parents shall be withheld unless a consent<br />

to release form (an AD 100 or equivalent), authorizing release <strong>of</strong> their identities <strong>and</strong><br />

signed by both parents, is filed in the case record. If only one parent signs the consent<br />

form all identifying information regarding the other parent must be withheld.<br />

The adopting family must also provide a signed release form (an AD 100 or equivalent)<br />

allowing the <strong>Department</strong> to release information about their family to the adoption agency<br />

providing the service.<br />

In order to complete an accurate <strong>and</strong> comprehensive adoptive home study the adoption<br />

agency completing the home study must be provided with the information given to the<br />

adopting parents regarding the child as well as information regarding the family that is<br />

adopting the child. The following information shall be considered for release:<br />

1. school records;<br />

2. child needs assessment records;<br />

3. routine medical/dental records;<br />

4. only the relevant information contained in the recommendations section <strong>of</strong> any<br />

psychological evaluation for the child;<br />

5. treatment plans for the child; <strong>and</strong><br />

6. court-ordered visitation plan for the child with his/her parents/guardians <strong>and</strong><br />

siblings, if any.<br />
