2.2 Family Reunification<br />

EXHIBIT A<br />

If the permanency plan is for family reunification, the CONTRACTOR shall assist<br />

the COUNTY in reunification efforts by: (1) placing the child with a home in<br />

his/her own neighborhood to the extent possible; (2) facilitating visits <strong>and</strong><br />

arranging transportation for the Placed Child with the family consistent with the<br />

orders <strong>of</strong> the court <strong>and</strong> the Needs <strong>and</strong> Services Plan; (3) <strong>of</strong>fering <strong>and</strong>/or<br />

supporting other reunification Services such as family counseling; (4) monitoring<br />

the visits with the family as needed. The CONTRACTOR shall Monitor for<br />

Compliance that the Certified Foster Parents cooperate with such reunification<br />

efforts <strong>and</strong> visitation.<br />

2.3 Adoption:<br />

2.3.1 Adoptions Assessments<br />

If the Placed Child’s permanency plan is for adoption, the CONTRACTOR<br />

shall participate with the CSW <strong>and</strong>/or Adoptions Worker to assess both the<br />

strengths <strong>and</strong> special needs <strong>of</strong> a Placed Child to assist in determining an<br />

appropriate adoptive home.<br />

2.3.2 Adoption-Related Services<br />

The CONTRACTOR shall facilitate the Placed Child's involvement in<br />

adoption-related activities <strong>and</strong> visits with prospective adoptive families.<br />

The CONTRACTOR shall provide counseling, support, <strong>and</strong> education for<br />

the Placed Child <strong>and</strong> his/her Certified Foster Parents in making decisions<br />

<strong>and</strong> transitions related to adoption or to any other legally permanent<br />

placement.<br />

2.3.3 Adoption-Related Information<br />

The COUNTY shall provide information, <strong>and</strong> the CONTRACTOR shall be<br />

fully informed, about the Adoption Assistance Program <strong>and</strong> the differences<br />

between legal guardianship, adoption, <strong>and</strong> foster care.<br />

2.4 Legal Guardianship:<br />

If the permanency plan is for legal guardianship, the CONTRACTOR shall assist<br />

the COUNTY by: (1) placing the child in the neighborhood <strong>of</strong> the legal guardian/<br />

proposed legal guardian to the extent possible; (2) facilitating visits <strong>and</strong> arranging<br />

transportation <strong>of</strong> the Placed Child with the legal guardian/proposed legal<br />

guardian consistent with the orders <strong>of</strong> the court <strong>and</strong> the Needs <strong>and</strong> Services<br />

Plan; (3) <strong>of</strong>fer <strong>and</strong> /or support other support Services such as family counseling<br />

to the legal guardian/ proposed legal guardian; (4) monitoring visits with the legal<br />

guardian/proposed legal guardians as needed. The CONTRACTOR shall<br />
