B. WHEN: A CHILD IS IN OUT-OF-HOME CARE<br />

Dependency Investigation SCSW or Case-Carrying SCSW Responsibilities<br />

EXHIBIT I<br />

1. Upon receipt <strong>of</strong> the Jurisdictional/Dispositional Hearing packet, make a copy <strong>of</strong> the<br />

JV 225 <strong>and</strong> provide it to the PHN. Retain the original JV 225 in the Court<br />

Documents folder <strong>and</strong> forward it to the assigned DI CSW or Case-Carrying CSW.<br />

See Procedural Guide 0300-503.12, Health <strong>and</strong> Education Questionnaire.<br />

2. For <strong>of</strong>fices who have implemented Concurrent Planning Redesign: The DI<br />

SCSW <strong>and</strong>/or case-carrying SCSW shall review the FB documents.<br />

For <strong>of</strong>fices who have not implemented Concurrent Planning Redesign: The DI<br />

SCSW <strong>and</strong>/or case-carrying SCSW shall review the DCFS 4344 I, II <strong>and</strong>/or III.<br />

NOTE: The PHN will enter the information from the JV 225 into the<br />

appropriate CWS/<strong>CMS</strong> Notebook.<br />

Case-Carrying FM&R CSW Responsibilities<br />

1. At the initial parent contact after the detention hearing:<br />

a) Obtain additional medical information from the parents as needed including the<br />

child’s immunization records, health care providers, allergies, chronic illnesses<br />

<strong>and</strong> other information needed to ensure that the child’s health needs are met.<br />

b) For <strong>of</strong>fices who have implemented Concurrent Planning Redesign Review<br />

FB1 <strong>and</strong> FB2, <strong>and</strong> add additional facts provided by the parents. Make sure that<br />

parents/caregiver has FB3 <strong>and</strong> give them a postage-paid envelop to mail it back<br />

to the <strong>of</strong>fice.<br />

For <strong>of</strong>fices who have not implemented Concurrent Planning Redesign.<br />

Review DCFS 4344 III, <strong>and</strong> add additional facts provided by the parents.<br />

2. Ensure that the JV 225 has been filed in the Court Documents folder. Review the<br />

child’s health <strong>and</strong> education information in CWS/<strong>CMS</strong> for completeness. See<br />

Procedural Guide 0300-503.12, Health <strong>and</strong> Education Questionnaire.<br />

3. Ensure that updated medical <strong>and</strong> family history information is entered into<br />

CWS/<strong>CMS</strong> in the applicable Notebooks.<br />

4. At each child contact, collect completed DCFS 561(a), (b) or (c) if the child had a<br />

medical/dental treatment, <strong>and</strong> review them with the caregiver for any needed followup.<br />
