5. Give the completed originals <strong>of</strong> the DCFS 561(a), (b) or (c) to the PHN who will enter<br />

the information into CWS/<strong>CMS</strong>.<br />

6. Confirm that all new medical, dental or psychological/other information noted on the<br />

DCFS 561(a), (b) or (c) has been entered into the applicable CWS/<strong>CMS</strong> Notebooks.<br />

7. Generate an updated HEP, <strong>and</strong> mail a copy to the caregiver.<br />

NOTE: Information regarding psychiatric diagnoses <strong>and</strong> psychotropic medications<br />

shall be included in the HEP <strong>and</strong> entered in the Health Notebook.<br />

Psychological/psychiatric evaluation reports are not to be provided to the caregiver <strong>and</strong><br />

must not be included in the HEP Binder. They shall be kept in the case file only.<br />

IQ scores shall not be entered in the child’s Client Notebook or otherwise entered in the<br />

HEP. Information relating to IQ shall be maintained in the Psychological/Medical/<br />

Dental/School Records folder only <strong>and</strong> used for the sole purpose <strong>of</strong> accessing<br />

resources such as Regional Center services.<br />

If there is no new medical, dental or psychological/other information between child<br />

contacts, an updated copy <strong>of</strong> the HEP need not be provided to the caregiver.<br />

8. When the school returns the DCFS 1726, provide the caregiver with a copy for<br />

inclusion in the HEP Binder <strong>and</strong> place the original in the<br />

Psychological/Medical/Dental/School Records folder.<br />

9. If DCFS 1726 indicates that the child is receiving special education services, obtain<br />

a copy <strong>of</strong> the child’s Individualized Education Program (IEP) from the school.<br />

Provide the caregiver with a photocopy for inclusion in the HEP Binder <strong>and</strong> place the<br />

original in the Psychological/Medical/Dental/School Records folder.<br />

10. Enter information regarding the IEP (if any), special education information (if any)<br />

<strong>and</strong> the principal’s name <strong>and</strong> phone number in the child’s Education Notebook.<br />

11. Review the child’s health <strong>and</strong> education information in CWS/<strong>CMS</strong> prior to preparing<br />

the Status Review Report.<br />

12. Generate an updated HEP. The updated HEP will automatically be populated with<br />

any health or education information entered since the previous HEP was generated.<br />
