equivalent providing the CONTRACTOR commits to maintain such coverage for<br />

not less than two years, or provide a two year extended reporting period,<br />

commencing upon termination or cancellation <strong>of</strong> this agreement. In all cases,<br />

such general liability coverage shall include limits <strong>of</strong> not less than the following:<br />

General Aggregate (if CONTRACTOR’s facility has a<br />

total licensed capacity <strong>of</strong> seven or more beds): $2 million<br />

General Aggregate (if CONTRACTOR’s facility has a<br />

total licensed capacity <strong>of</strong> six or less beds): $1 million<br />

Products/Completed Operations Aggregate: $1 million<br />

Personal <strong>and</strong> Advertising Injury: $1 million<br />

Each Occurrence: $1 million<br />

Note: General Aggregate limits for Subcontractors shall be not less than $1<br />

million.<br />

6.2 Automobile Liability insurance (written on ISO policy form CA 00 01 or its<br />

equivalent) with a limit <strong>of</strong> liability <strong>of</strong> not less than one million dollars<br />

($1,000,000) for each accident. Such insurance shall include coverage for all<br />

“owned,” “hired” <strong>and</strong> “non-owned” vehicles, or coverage for “any auto,” <strong>and</strong><br />

include a deductible no greater than $1,000 in accordance with <strong>County</strong> Code<br />

(Section 2.38.060).<br />

6.3 Workers’ Compensation <strong>and</strong> Employer’s Liability insurance providing workers’<br />

compensation benefits, as required by the Labor Code <strong>of</strong> the State <strong>of</strong> California<br />

or by any other state, <strong>and</strong> for which CONTRACTOR is responsible. If<br />

CONTRACTOR’s employees will be engaged in maritime employment,<br />

coverage shall provide workers’ compensation benefits as required by the U.S.<br />

Longshore <strong>and</strong> Harbor Workers’ Compensation Act, Jones Act or any other<br />

federal law for which CONTRACTOR is responsible.<br />

In all cases, the above insurance also shall include Employers’ Liability<br />

coverage with limits <strong>of</strong> not less than the following:<br />

Each Accident: $1 million<br />

Disease – policy limit: $1 million<br />

Disease – each employee: $1 million<br />

6.4 Pr<strong>of</strong>essional Liability: Insurance covering liability arising from any error,<br />

omission, negligent or wrongful act <strong>of</strong> CONTRACTOR, its <strong>of</strong>ficers or employees<br />

with limits <strong>of</strong> not less than one million dollars ($1,000,000) per occurrence <strong>and</strong><br />

two million dollars ($2,000,000) aggregate if CONTRACTOR’s facility has a total<br />

licensed capacity <strong>of</strong> seven or more beds <strong>and</strong> one million dollars ($1,000,000)<br />

aggregate if CONTRACTOR’s facility has a total licensed capacity <strong>of</strong> six or less<br />

beds. The coverage also shall provide an extended two-year reporting period<br />

commencing upon termination or cancellation <strong>of</strong> this Contract, unless an<br />

occurrence policy is in place with coverage for prior acts.<br />
