The CONTRACTOR shall provide the Certified Foster Parents with copies<br />

<strong>of</strong> updated relevant records when received from DCFS for inclusion in the<br />

Black Binder, or the equivalent.<br />

3.8.7 Medications <strong>and</strong> Court Authorizations at Replacement<br />

At the time <strong>of</strong> a child’s replacement, the CONTRACTOR shall give any<br />

medications <strong>and</strong> court authorizations for the administration <strong>of</strong> psychotropic<br />

drugs to the CSW. If the medications <strong>and</strong> court authorizations are not<br />

available at the time <strong>of</strong> replacement outside the agency, CONTRACTOR<br />

shall send them to the CSW within 24 hours <strong>of</strong> the replacement.<br />

3.9 Health <strong>and</strong> Medical Monitoring Requirements:<br />

3.9.1 Immunizations <strong>and</strong> Routine Health Care<br />

The CONTRACTOR shall monitor the immunization <strong>and</strong> routine health<br />

care status <strong>of</strong> all Placed <strong>Children</strong> <strong>and</strong> shall accurately reflect this<br />

information in the Placed Child’s medical records folder or Health Portion<br />

<strong>of</strong> the Health <strong>and</strong> Education Passport when provided by DCFS.<br />

3.9.2 Medications<br />

The CONTRACTOR shall Monitor for Compliance that Certified Foster<br />

Parents record type, date, <strong>and</strong> time <strong>of</strong> all prescription <strong>and</strong> non-prescription<br />

medication administered to the Placed Child.<br />

3.9.3 Psychiatric Care <strong>and</strong> Clinical Evaluations by DMH Approved Providers<br />

The CONTRACTOR shall Monitor for Compliance that psychiatric needs<br />

<strong>of</strong> the Placed Child are met in accordance with the CHDP program, Medi-<br />

Cal program, <strong>and</strong> CCLD regulations to the extent that funding <strong>and</strong><br />

services are available.<br />

The CONTRACTOR shall: (1) take all necessary steps to ensure that any<br />

Placed Child in its care with a known history <strong>of</strong> psychiatric problems<br />

(including hospitalizations) receives a clinical evaluation, provided that<br />

such an evaluation is authorized by DMH, conducted by a licensed mental<br />

health pr<strong>of</strong>essional; <strong>and</strong> (2) submit to the CSW the written results <strong>of</strong> such<br />

tests when obtained by the CONTRACTOR.<br />

3.9.4 Assessment, Continuing Evaluation, <strong>and</strong> the Required Court<br />

Authorizations on Psychotropic Medication<br />

The CONTRACTOR shall Monitor for Compliance that Placed <strong>Children</strong> on<br />

psychotropic medication have a psychiatric/psychological assessment,<br />

indicating the Placed Child's diagnosis, need for treatment, prognosis, <strong>and</strong><br />

possible side effects <strong>of</strong> the medication. The CONTRACTOR shall arrange<br />
