SAMPLE CONTRACT<br />

<strong>and</strong>/or Certified Foster Parent is certified or de-certified. Notice to<br />

COUNTY Program Manager is required at the end <strong>of</strong> the month prior to<br />

prospective certification <strong>of</strong> a new certified home. Notification <strong>of</strong> certification<br />

shall occur at least 72 hours prior to placement. Notice <strong>of</strong> decertification<br />

shall occur within 72 hours following the date <strong>of</strong> de-certification <strong>and</strong> shall<br />

include the name <strong>of</strong> the foster parent, date <strong>of</strong> birth <strong>and</strong> social security<br />

number. These notices shall be sent to the COUNTY Program Manager<br />

<strong>and</strong> Revenue Enhancement. Decertification notification shall include the<br />

reason for decertifying. Failure to provide this information to COUNTY may<br />

result in a Do Not Refer Status being placed on the FFA.<br />

12.9 COUNTY shall notify CONTRACTOR <strong>of</strong> its intent to place child(ren) in a<br />

home <strong>of</strong> a relative or extended family member in a Certified Family Home.<br />

12.10 Once a guardianship is finalized or an adoptive placement <strong>of</strong> a child in a<br />

certified home occurs, COUNTY shall notify CONTRACTOR.<br />

12.11 CONTRACTOR shall report all Certified Family Homes <strong>and</strong>/or Certified<br />

Foster Parent(s) who are decertified (<strong>and</strong> the reason for decertification),<br />

since those recorded on the previous month’s report on the Foster Family<br />

Agency Monthly Report (Exhibit A-XII), as described in Part I, Sub-section<br />

18.6, Program Reporting Requirements, here<strong>of</strong>.<br />

12.12 In the event that a CONTRACTOR is put on Do Not Use Status, to ensure<br />

continuity <strong>of</strong> care for Placed <strong>Children</strong>, COUNTY may continue placement in<br />

the Certified Family Home if the Certified Foster Parent applies for<br />

licensure by the State, becomes certified by another FFA, or DCFS<br />

approves the home as a non-relative extended family member foster home.<br />

13.0 STATE LICENSE<br />

14.0 FEES<br />

13.1 The CONTRACTOR shall maintain a FFA license issued by the California<br />

<strong>Department</strong> <strong>of</strong> Social Services, CCL Division, throughout the term <strong>of</strong> the<br />

Contract. A copy <strong>of</strong> the current license shall be included in the Program<br />

Statement.<br />

13.2 The CONTRACTOR shall provide Services pursuant to the approved<br />

Program Statement. If planning to add additional <strong>of</strong>fices during the term <strong>of</strong><br />

the Contract, the CONTRACTOR shall notify the COUNTY Program<br />

Manager <strong>and</strong> must obtain written approval prior to the placement <strong>of</strong> <strong>and</strong>/or<br />

serving Placed <strong>Children</strong> from the additional <strong>of</strong>fice(s). Contractor’s decision<br />

to pursue licensing <strong>of</strong> additional <strong>of</strong>fices from CCLD does not ensure<br />

placements from <strong>Los</strong> <strong>Angeles</strong> <strong>County</strong>.<br />
