6.0 PROFESSIONAL TREATMENT TEAM:<br />

6.1 Purpose <strong>of</strong> the Treatment Team:<br />

The CONTRACTOR shall provide a pr<strong>of</strong>essional on-site treatment team<br />

that specifically defines how every adult having contact with the Placed<br />

Child will intervene to help the Placed Child overcome the problems <strong>and</strong><br />

achieve the goals specified in the Needs <strong>and</strong> Services Plan portion <strong>of</strong> the<br />

Needs <strong>and</strong> Services Plan/Quarterly Report template (Exhibit A-V). The<br />

purpose <strong>of</strong> the treatment team is to coordinate this plan so that each adult<br />

having contact with the child fully underst<strong>and</strong>s the Plan, his/her part in it,<br />

<strong>and</strong> the nature <strong>of</strong> his/her intervention with the Placed Child.<br />

6.2 Persons Included in the Treatment Team:<br />

The treatment team shall be led by the CONTRACTOR’S social worker or<br />

mental health pr<strong>of</strong>essional in charge <strong>of</strong> developing Needs <strong>and</strong> Services<br />

Plans/Quarterly Reports. It shall also include the facility managers, the<br />

childcare <strong>and</strong> supervision staff, the Placed Child, <strong>and</strong> when appropriate,<br />

the family members.<br />

6.3 Duties <strong>of</strong> the Treatment Team:<br />

The treatment team in collaboration with either the <strong>County</strong> Worker or the<br />

<strong>County</strong> Worker’s Supervisor shall: (1) develop a comprehensive<br />

individualized Needs <strong>and</strong> Services Plan within 30 Days <strong>of</strong> the date <strong>of</strong> initial<br />

placement that contains goals that while treating the identified needs <strong>of</strong><br />

the Placed Child is outcome-based, specific, measurable, attainable, <strong>and</strong><br />

has a specific time frame for each deliverable; (2) incorporate the content<br />

required in Title 22, Division 6, Chapter 5, Sections 84068.2(b-c) <strong>and</strong><br />

84068.3(a), including the plans for health <strong>and</strong> education, visitation, types<br />

<strong>of</strong> Services necessary including treatment, strengths <strong>of</strong> the Placed Child<br />

<strong>and</strong> his/her family; (3) incorporate the content required from the PROB<br />

1385 or the DCFS 709; (4) determine <strong>and</strong> communicate the role <strong>of</strong> each<br />

person having contact with the Placed Child to enact the Needs <strong>and</strong><br />

Services Plan; (5) determine the Placed Child’s progress or lack <strong>of</strong><br />

progress, including in independent living skills, <strong>and</strong> adjust the Needs <strong>and</strong><br />

Services Plan accordingly; <strong>and</strong> (6) discuss <strong>and</strong> formulate the behavior<br />

management <strong>and</strong> intervention plans to which each Placed Child best<br />

responds.<br />

For Probation youth, the Needs <strong>and</strong> Service Plan shall address the<br />

criminogenic needs as identified in the DPO’s (Deputy Probation Officer)<br />

assessment <strong>and</strong> outline specific goals that target the identified<br />

criminogenic needs. All goals must be measurable.<br />
