Placed <strong>Children</strong>; <strong>and</strong> (2) abide by all applicable federal <strong>and</strong> state laws <strong>and</strong><br />

regulations in transporting Placed <strong>Children</strong>.<br />

The CONTRACTOR shall monitor <strong>and</strong> maintain records to verify that staff who<br />

transport the Placed <strong>Children</strong>: (1) have <strong>and</strong> maintain a valid driver’s license with<br />

the <strong>Department</strong> <strong>of</strong> Motor Vehicles; <strong>and</strong> (2) insure their vehicles, if used to<br />

transport the Placed <strong>Children</strong>, in compliance with the insurance coverage<br />

requirements set forth in the Master Contract, Section 5.2.<br />

1.4 CONTRACTOR’S Responsibilities for Placed <strong>Children</strong> Off Grounds:<br />

1.4.1 Pre-Approval by <strong>County</strong> Worker:<br />

For DCFS, Placed <strong>Children</strong> may leave the facility unaccompanied for<br />

specific purposes if it has been pre-approved by the <strong>County</strong> Worker in the<br />

Needs <strong>and</strong> Services Plan/Quarterly Report template <strong>and</strong> the<br />

CONTRACTOR or designee agrees. The CONTRACTOR staff shall know<br />

the whereabouts <strong>of</strong> Placed <strong>Children</strong> who are <strong>of</strong>f grounds <strong>and</strong> be able to<br />

identify who is responsible for supervision at all times.<br />

For Probation, Placed <strong>Children</strong> shall be supervised at all times unless<br />

otherwise specifically stated in the current Probation Foster Care Case<br />

Plan <strong>and</strong> the COUNTY approved Needs <strong>and</strong> Service Plan developed by<br />

the CONTRACTOR’S Treatment Team. If a Probation minor leaves a<br />

facility without authorization, the CONTRACTOR shall contact the Police<br />

<strong>Department</strong>, complete a police report, <strong>and</strong>, if event occurs within normal<br />

working hours, immediately notify Central Placement’s OHC Unit Officer <strong>of</strong><br />

the Day. If the event occurs after hours, the CONTRACTOR shall submit<br />

an I-Track report <strong>and</strong> notify the DPO <strong>of</strong> record the next business day.<br />

1.4.2 Maintenance <strong>of</strong> a Sign-in/Sign-out Log:<br />

The CONTRACTOR shall maintain a detailed sign-in/sign-out log for<br />

Placed <strong>Children</strong> who leave the facility for any reason other than regularly<br />

scheduled work, school, or group activities <strong>of</strong> the CONTRACTOR. This<br />

log shall include the name <strong>of</strong> the child, his/her destination, the time he/she<br />

left the facility, the anticipated time <strong>of</strong> return, <strong>and</strong> the name <strong>and</strong> telephone<br />

number <strong>of</strong> the person who is responsible to supervise the resident while<br />

he/she is away from the facility.<br />

The CONTRACTOR shall maintain a daily log <strong>of</strong> all visitors that includes<br />

the following information: (1) the <strong>County</strong> Worker; (2) the person they are<br />

visiting; <strong>and</strong> (3) the arrival <strong>and</strong> departure times. For Probation, the<br />

CONTRACTOR shall ensure that all DPOs (except On-Site DPOs) <strong>and</strong><br />

Regional DPOs that supervise youth at more than one site sign the entry<br />

log prior to his/her visitation.<br />

1.5 Restraints <strong>and</strong> Seclusion:<br />
