SAMPLE CONTRACT<br />

2.27 “Foster Care Funding <strong>and</strong> Rates Bureau” – means the Division <strong>of</strong> the<br />

California <strong>Department</strong> <strong>of</strong> Social Services that establishes Aid to Families<br />

with Dependent <strong>Children</strong>-Foster Care (AFDC-FC) rates for Foster Family<br />

Agencies.<br />

2.28 “Foster Care Payment Hotline” – means a telephone number that<br />

CONTRACTOR may call under circumstances described in this Contract<br />

(i.e., within 24 hours <strong>of</strong> child leaving the home) or may call to request<br />

payment or Medi-Cal information. The Foster Care Payment Hotline<br />

Number is (800) 697-4444.<br />

2.29 “Foster Family Agency” or “FFA” – means any organization that, in<br />

compliance with Title 22, Division 6, Chapters 4 <strong>and</strong> 8.8, engaged in the<br />

recruiting, certifying, <strong>and</strong> training <strong>of</strong>, <strong>and</strong> providing pr<strong>of</strong>essional support to,<br />

Certified Foster Parent(s), or in finding homes for placement <strong>of</strong> children for<br />

temporary or permanent care who require that level <strong>of</strong> care as an<br />

alternative to a group home. Private foster family agencies shall be<br />

organized <strong>and</strong> operated on a nonpr<strong>of</strong>it basis.<br />

2.30 “Foster Family Agency Program Rates” – means the service rate levels<br />

payable to FFAs, as periodically established by the <strong>Department</strong> <strong>of</strong> Social<br />

Services, Foster Care Funding <strong>and</strong> Rates Bureau.<br />

2.31 “Health <strong>and</strong> Education Passport” or “Black Binder (DCFS)” means the<br />

Health <strong>and</strong> Education Passport that is the summary <strong>of</strong> the health (including<br />

dental <strong>and</strong> mental health information) <strong>and</strong> educational information required<br />

by Welfare <strong>and</strong> Institutions Code Section 16010 (Exhibit I) that is to follow<br />

the child to all foster placements. DCFS created nylon Black Binder<br />

divided into three sections. The first two sections, “Medical <strong>and</strong> Dental<br />

Information” <strong>and</strong> “Educational Information,” meet the requirements <strong>of</strong><br />

Section 16010. The third section, “Placement Documentation,” contains<br />

additional items such as photographs <strong>of</strong> the child <strong>and</strong> his or her family,<br />

birth <strong>and</strong> death certificates, pro<strong>of</strong> <strong>of</strong> Medi-Cal eligibility, <strong>and</strong> the CSW’s<br />

business card. (DCFS may change the Health <strong>and</strong> Education Passport<br />

format in the future).<br />

2.32 “Hold Status” – means a temporary suspension <strong>of</strong> referrals <strong>of</strong> children to<br />

CONTRACTOR by placing CONTRACTOR on Hold Status for up to a 45day<br />

period at any time during investigations, as further defined in Part I,<br />

Section 16.0 Hold Status, Do Not Refer Status, Do Not Use Status,<br />

Corrective Action Plan, Sub-section 16.2 <strong>of</strong> this Contract, <strong>and</strong> Exhibit N,<br />

DCFS Foster Family Agency Contract Investigation/Monitoring/Audit<br />

remedies <strong>and</strong> Procedures.<br />

2.33 “Independent Living Program” or “ILP” – means the program authorized<br />

under 42 U.S.C. 677 <strong>of</strong> the Social Security Act for services <strong>and</strong> activities to<br />
