EXHIBIT A<br />

(a) For 0-6 months, families should visit at least three times a week for<br />

30-60 minutes.<br />

(b) For 6-12 months, families should visit at least three times a week for<br />

one hour.<br />

(c) For 1-4 years, families should visit at least twice per week for 1 ½<br />

hours.<br />

(d) For 5-15 years, families should visit at least once per week for two or<br />

more hours.<br />

(e) For 16-18 years, there is no recommendation except the child’s<br />

desires should be strongly considered in creating the FVP<br />

3.2.5 Honoring the Visitation Rights <strong>of</strong> <strong>Children</strong><br />

The CONTRACTOR shall honor the visitation rights <strong>of</strong> the Placed Child at<br />

all times unless one <strong>of</strong> the following two conditions exists: (1) a specified<br />

court order is in effect which prohibits or restricts the visitation rights <strong>of</strong> the<br />

child; or (2) the FVP developed by the TEAM specifically prohibits or<br />

restricts visitation rights based upon existing court orders, legal authority<br />

<strong>and</strong>/or documented reasons such as the belief that the visits would be<br />

detrimental to the child.<br />

The CONTRACTOR must allow visitation for the caseworker, attorney,<br />

<strong>and</strong> Court Appointed Special Advocate (CASA). The CSW shall provide<br />

CONTRACTOR with copies <strong>of</strong> court orders regarding court ordered<br />

visitation (Exhibit G) <strong>and</strong> the FVP.<br />

The FVP shall be discussed with the Certified Foster Parent to facilitate<br />

his/her underst<strong>and</strong>ing <strong>of</strong> COUNTY’S <strong>and</strong> the CONTRACTOR’S<br />

expectations <strong>of</strong> the Certified Foster Parent, including the transportation<br />

arrangements <strong>and</strong> the birth parent/guardian visiting the Placed Child in the<br />

Certified Family Home when applicable. A copy <strong>of</strong> the FVP shall be given<br />

to the Certified Foster Parent.<br />

3.3 Needs <strong>and</strong> Services Plan <strong>and</strong> Related Social Services:<br />

3.3.1 Required Content <strong>of</strong> the Needs <strong>and</strong> Services Plan<br />

The CONTRACTOR’S social worker shall develop a comprehensive,<br />

individualized Needs <strong>and</strong> Services Plan, based upon the information<br />

provided by the CSW <strong>and</strong> CONTRACTOR’S initial assessment, within 30<br />

Days <strong>of</strong> placement that: (1) contains both long-term <strong>and</strong> short-term goals<br />

that treat the identified needs <strong>of</strong> the Placed Child; (2) is specific,<br />

measurable, attainable, <strong>and</strong> time-limited; <strong>and</strong> (3) meets the requirements<br />

specified in Title 22, Division 6, Chapter 8.8, Sections 88<strong>07</strong>0, 88<strong>07</strong>0.1,<br />
