Dependency Investigator CSW Responsibilities<br />

EXHIBIT I<br />

1. Prior to preparing the Jurisdiction/Disposition Hearing Report, ensure that the JV<br />

225 has been filed in the Court Documents folder. Review the child’s health <strong>and</strong><br />

education information in CWS/<strong>CMS</strong> for completeness. See Procedural Guide 0300-<br />

503.12, Health <strong>and</strong> Education Questionnaire. Information should include the child’s<br />

initial medical <strong>and</strong> dental examinations, immunization record, any medical conditions<br />

or allergies, family medical history <strong>and</strong> educational information.<br />

2. Ensure that missing information is entered into CWS/<strong>CMS</strong> as soon as it becomes<br />

available.<br />

3. Generate an updated HEP. Attach the HEP <strong>and</strong> the JV 225 to the<br />

Jurisdiction/Disposition Court Report.<br />

C. WHEN: A CHILD IS REPLACED<br />

Case-Carrying CSW Responsibilities<br />

1. Whenever a child is replaced, the HEP Binder, including an updated HEP, must<br />

accompany the child.<br />

2. Give the HEP Binder to the new caregiver at the time <strong>of</strong> the child’s replacement.<br />

a) If someone other than the case-carrying CSW transports the child to the new<br />

placement (e.g., law enforcement or the ERCP CSW), the case-carrying CSW<br />

shall provide the HEP Binder, including the updated HEP, within three business<br />

days to the new caregiver.<br />

NOTE: If the child is moved from one Foster Family Agency (FFA) certified foster<br />

family home to another certified foster family home within the same FFA, the CSW shall<br />

provide the HEP Binder to the new caregiver within three business days.<br />

3. When required, ensure that the child is medically treated prior to replacement. Bring<br />

the hospital/physician aftercare instructions, along with any prescribed medication,<br />

to the new placement. Place these documents in the HEP Binder <strong>and</strong> copies in the<br />

Psychological/Medical/ Dental/School Records folder. See Procedural Guide 0600-<br />

506.10, Child Health <strong>and</strong> Disability Prevention (CHDP) Program.<br />

4. Ensure that the health care provider completes the DCFS 561(a) <strong>and</strong> document the<br />

nature <strong>of</strong> the illness or injury, date <strong>of</strong> treatment <strong>and</strong> treatment provided, in the<br />

appropriate CWS/<strong>CMS</strong> Notebook(s).<br />

D. WHEN: THE COURT TERMINATES JURISDICTION<br />
