Child’s permanency plan as quickly as possible; (3) implements permanency<br />

plans in family settings; <strong>and</strong> (4) if the permanency plan is for out-<strong>of</strong>-home<br />

placement, wraps the necessary services around the Placed Child to ensure<br />

placement success <strong>and</strong> prevent replacements.<br />

Consistent with these actions, the COUNTY has included in this SOW the<br />

following requirements regarding: (1) Certified Foster Parent training [Section C<br />

3.6]; (2) permanency planning [Section C 2.1]; (3) adoption [Section C 2.3];<br />

(4) sibling-group placements [Section C 2.1.6]; <strong>and</strong> (5) neighborhood/schoolbased<br />

placements [C 3.3.8].<br />

5.0 SERVICE DELIVERY SITES<br />

CONTRACTOR’S Services shall be at the locations specified on Exhibit W,<br />

Service Delivery Sites.<br />

CONTRACTOR shall request approval from the DCFS Out <strong>of</strong> Home Care<br />

Management (OHCM) Division Chief or designee in writing a minimum <strong>of</strong> thirty<br />

(30) Days before: (1) terminating Services at any <strong>of</strong> the above location(s); <strong>and</strong><br />

(2) before commencing Services at any other location(s) not previously approved<br />

in writing by the DCFS (OHCM) Division Chief or designee.<br />
