Antiparkinsonian <strong>Drug</strong>s<br />

Ropinirole<br />

I: Parkinson’s disease, either used alone or as an adjunct<br />

to levodopa.<br />

C/I : Pregnancy and breast-feeding.<br />

P/C : Hepatic impairment, renal impairment; severe<br />

cardiovascular disease, major psychotic disorders, avoid<br />

abrupt withdrawal.<br />

A/E: Nausea, drowsiness, leg edema, abdominal pain,<br />

vomiting and syncope; dyskinesia, hallucinations and<br />

confusion reported in adjunctive therapy; occasionally<br />

severe hypotension and bradycardia.<br />

P/A: Tablets, 0.25mg, 0.5mg, 1mg, 2mg<br />

Dose: Oral, initially 750mcg daily in 3 divided doses, increased<br />

by increments of 750mcg at weekly intervals to 3mg<br />

daily; further increased by increments of upto 3mg at<br />

weekly intervals according to response; usual range 3-<br />

9mg daily; maximum 24mg daily.<br />

Pramipexole<br />

I: Parkinson disease either alone or as an adjunct to<br />

levodopa therapy to reduce the end of dose or on-off<br />

fluctuations in response. Moderate to severe restless<br />

leg syndrome.<br />

P/C: Used with caution in patients with renal impairment<br />

and regular ophthalmologic monitoring for visual<br />

impairment.<br />

A/E: GI disturbances, headache, dizziness, postural<br />

hypotension, drowsiness, vasospasm of fingers and toes<br />

particularly in-patients with Raynaud’s syndrome,<br />

pathological gambling.<br />

Dose: Initial dose 125mcg tds increased to 250 -500mcg tds<br />

according to response to a maximum dose of 4.5mg<br />

daily.<br />

D/I: Sedating drugs and alcohol used with pramipexole leads<br />

to additive effects and sudden onset of sleep.<br />
