acidosis; severe peripheral arterial disease;<br />

phaeochromocytoma (unless used with alpha-blocker)<br />

Avoid abrupt withdrawal especially in ischaemic heart<br />

disease;history of obstructive airway disease (use with<br />

caution and monitor lung function—see also<br />

contraindications above); pregnancy breastfeeding<br />

first-degree atrioventricular block; liver function<br />

deteriorates in portal hypertension; reduce dose in<br />

renal impairment, diabetes mellitus (small decrease in<br />

glucose tolerance, masking of symptoms of<br />

hypoglycaemia).<br />

Tablet: 50 mg; 100 mg.<br />

Angina, by mouth, 50 mg once daily, increased if<br />

necessary to 50 mg twice daily or 100 mg once daily<br />

Myocardial infarction (early intervention within 12 hours), by<br />

intravenous injection over 5 minutes, 5 mg, then by mouth 50 mg after 15<br />

minutes,followed by 50 mg after 12 hours, then 100 mg daily<br />

A/E: gastrointestinal disturbances (nausea, vomiting,<br />

diarrhoea,constipation, abdominal cramp); fatigue;<br />

bronchospasm; bradycardia, heart failure, conduction<br />

disorders, hypotension; sleep disturbances, including<br />

nightmares; depression, confusion; hypoglycaemia or<br />

hyperglycaemia.<br />

CALCIUM-CHANNEL BLOCKERS<br />

A long-acting dihydropyridine calcium channel blocker (such as<br />

amlodipine) can be added to betablocker treatment if necessary for control<br />

of moderate stable angina. For those in whom a beta-blocker is inappropriate,<br />

verapamil may be given as an alternative to treat stable angina. Calciumchannel<br />

blockers can also be used in patients with unstable angina with a<br />

vasospastic origin, such as Prinzmetal angina, and in patients in whom<br />

alterations in cardiac tone may influence the angina threshold<br />

MISCELLANEOUS DRUGS USED IN TREATMENT OF ANGINA<br />

Trimetazidine<br />

It is an antianginal agent which acts by vasodilation and effects on<br />

cardiac metabolism
